No adverse event/ no known pt adverse events; Indavertently administered improperly stored GARDASIL 9/ Temps went out of range. Vaccines given despite excursion.; This spontaneous report was received from a licensed practical nurse (LPN) regarding to a 15-year-old male patient. The patient's concurrent conditions, pertinent medical history, concomitant therapies, drug reactions, and allergies were not provided. On 03-NOV-2021, the patient was vaccinated, for prophylaxis, intramuscularly with an improperly stored (product storage error) dose of HPV rL1 6 11 16 18 31 33 45 52 58 VLP vaccine (yeast) (GARDASIL 9) lot number T028929, with an expiration date of 08-MAR-2023 (dose, strength and formulation were not reported), subcutaneously with the dose of varicella virus vaccine live (Oka/Merck) (VARIVAX) lot number U010396), with an expiration date of 01-APR-2023 (also reported as 28-SEP-2022, upon internal validation, this is an incorrect expiry date) (dose, strength and formulation were not reported), subcutaneously with the dose of measles, mumps, and rubella (wistar ra 27-3) virus vaccine, live (recombinant human albumin) (M-M-R II) lot number T020655, with an expiration date of 16-JUN-2022 (also reported as 26-MAR-2022, upon internal validation, this is an incorrect expiry date) (dose, strength and formulation were not reported), intramuscularly with the dose of hepatitis a vaccine inact (HAVRIX), lot number 2JD3Y, with expiration date of 04-DEC-2022 (dose, strength and formulation were not reported), intramuscularly with the dose of diphteria vaccine toxoid, hepatitis b vaccine rhbsag (yeast), pertussis vaccine acellular 3-component, polio vaccine inact 3v (vero), tetanus vaccine toxoid (PEDIARIX), lot number J7H4D , with expiration date of 23-MAR-2022 (dose, strength and formulation were not reported), intramuscularly with the first dose of meningococcal vaccine a/c/y/w conj (diptox)(MENACTRA), lot number U7190AA, with expiration date of 16-JAN-2023 (dose, strength and formulation were not reported), intramuscularly with the dose of diphtheria vaccine toxoid, pertussis vaccine acellular 3-component, tetanus vaccine toxoid (BOOSTRIX), lot number C5863AA, with expiration date of 13-APR-2023 (dose, strength and formulation were not reported). There were no additional adverse event details. Administered dose of vaccine experienced the temperature excursion of 27.2 degrees Fahrenheit and 49.8 degrees Fahrenheit for 8 hours 30 minutes and 21 hours and 15 minutes respectively. There were no previous excursions reported. The outcomes of all the events for all the vaccines administered was not reported. Action taken for all the vaccines was not applicable. The causality assessment between all the adverse events and all of the vaccines administered was not applicable.
