Patient left facility after shot, even though she signed to acknowledge that we recommend a 15 minute wait. Patient had an episode of syncope when being driven home by her husband.

Symptomtext

22 days following the vaccination, I noticed two lesions on my forehead which I attributed to mosquito bites. I felt fatigued and had a slight headache, but did not pay much attention to that. The next day, I noticed a few more smaller spots show up on my face, and over the weekend I had an itchy scalp. Monday morning I had 4 lesions located centrally on the sternum, but at this time I still believed these to be mosquito bites. During the afternoon, I began to feel more fatigued and felt generally unwell. I was getting itchier at this time as well. Upon returning from work and getting ready for a shower, I noticed the lesions all over my trunk, and beginning to show up on me extremities. The lesions are typical of chicken pox, which are maculopapular with vesicles. The diagnosis was confirmed by my primary care. New lesions no longer seem to be forming, and I estimate to have 50-60. I have quarantined since being aware of my infection on Monday.

Patient came into office for an annual well child check, patient was due for three vaccines: Hep A, MMR, and VAR. MA pulled MMRV instead of MMR and was administered to patient as well as HAVRIX. Patient received two doses of varicella on 12/27/22. Since the administration, MA contacted manager and provider to make them aware of the situation. Patient will be monitored closely by mother.

After the patient left the clinic while completing the vaccination record paperwork, this RN realized the Bexero vaccine administered to the patient had an expiration date of 07/31/22. Supervisor was immediately notified. Call placed to Glaxo Smith-Kline Medical Information to find out vaccine viability period past the expiration date. Spoke to Medical Information Scientist who informed this RN that although GSK does not recommend, and the CDC has not evaluated the immunogenicity or safety of giving an expired vaccine, an in-house stability analysis and potency testing study shows that when stored under proper conditions and temperatures, the Bexero vaccine remains within approved specification up to 30 days past the expiration date. The Bexero Vaccine given had been continuously stored in the proper conditions and temperatures prior to being administered. This RN requested writtten documentation of information received over the telephone, and received the information via email. No adverse reactions were reoprted by the patient or parent.
