Patient received above listed vaccines in clinic without difficulties at approximately 2:45pm with mom and uncle present in room. Patient was sent to lobby for 10 minutes of monitoring. At 3:00pm front office staff came to nurses room stating patient had passed out and was carried to vehicle by his uncle. Nurse to patient vehicle, alert and oriented x4. vital signs stable. patient stated he began feeling weak and sick to his stomach and then passed out. Stated he now feels "normal".

vaccine placed on 11/27, on 12/8 pt states she noted redness, swelling and soreness to right posterior arm where varivax vaccine administered. Took tylenol to relieve the pain which helped. Also notes 2 days later felt pain radiating to left shoulder as well. This resolved the next day. Seen on 12/11 at clinic and redness, soreness, and swelling to posterior arm continues but is improved.

Temperature 6?F; This spontaneous report as received from a registered nurse and an administrator, referring to a 26 years old female patient. Concomitant therapies included sterile diluent. The patient's historical condition and current condition were not reported. It was reported that multiple patients received vaccines that underwent the temperature excursion prior to the office being aware of it, between 08-APR-2022 to 02-MAY-2022. On 27-APR-2022, the patient was vaccinated with incorrectly storge of varicella virus vaccine live (oka/merck) (VARIVAX) (lot # U010398 with expiration date: 01-APR-2023; dose, route and strength were unknown) for prophylaxis (product storage error). Administered doses of varicella virus vaccine live (oka/merck) (VARIVAX) experienced the temperature excursion below 6 Fahrenheit degrees (F) up to 14F (7.9F) for 4 hours 8 minutes 0 seconds. The patient's medical history, historical drugs, concurrent conditions and concomitant medications were not provided. On 17-SEP-2021, the patient was going to be vaccinated with varicella virus vaccine live (oka/merck)(VARIVAX) 0.5 milliliter, lot number U010398, expiration date 01-APR-2023 (route of administration not provided) for prophylaxis, however, the vaccine was stored at room temperature (72 degrees Fahrenheit) for 1 hour and 25 minutes. Due to this, the vaccine was not administered. No adverse event was reported.

No other adverse events reported.; Caller reports that an improperly stored dose of VARlVAX administered to patient on 25Aug2021; This spontaneous report was received from a registered nurse referring to a patient of unknown age and gender. The patient's pertinent medical history, concurrent conditions, previous drug reactions, allergies, or concomitant medications were not reported. On 25-AUG-2021, the patient was vaccinated with an improperly stored varicella virus vaccine live (Oka/Merck) (VARIVAX) injection, 1 dose as prophylaxis (lot number U010398 which has been verified to be valid, expiration date reported and validated as 01-APR-2023) (route of administration and anatomical site of injection were not reported) (product storage error). The administered dose of the vaccine was stored at a temperature of -13.5 degrees Celsius (C) (also reported as -14 C to -10 C (-13.5 C)), during a time frame of 6 hours 39 minutes. There was no previous temperature excursion. No other adverse events were reported in the patient.
