I noticed one of the veins in my underarm was swollen in th same arm/ I have a blood clot i my upper right arm/ have to be evaluated for DVT; I noticed one of the veins in my underarm was swollen in th same arm/ I have a blood clot in my upper right arm/ have to be evaluated for DVT; This spontaneous report was received from a female patient of unknown age reporting on herself, via a company representative. The patient's concurrent conditions, pertinent medical history, drug reactions or allergies, and concomitant therapies were not reported. On 23-FEB-2022, the patient was vaccinated with pneumococcal vaccine, polyvalent (23-valent) (PNEUMOVAX23) 0.5 ml syringe, lot number U010534 with expiration date 31-OCT-2022, in the right arm for prophylaxis (route of administration was not reported). The patient reported that on 04-MAR-2022 she noticed one of the veins in her underarm was swollen in the same arm of the vaccine. On 09-MAR-2022, she went to see the primary-care physician (PCP), who told her that she had a blood clot (Thrombosis) in her upper right arm and that this should be evaluated for deep vein thrombosis (DVT). The outcome of thrombosis and deep vein thrombosis was not reported. The causal assessment between the events and pneumococcal vaccine, polyvalent (23-valent) (PNEUMOVAX23) was not provided. The PCP believed that it should be reported as an adverse reaction to the pneumococcal vaccine. Upon internal review, the events of thrombosis and deep vein thrombosis were determined to be medically significant.

Patient, called the pharmacy on 10/27/2021 around 1:05- 1:10pm. She received both the Pneumovax 23 and Moderna booster on Saturday 10/23/2021. Patient stated that on Monday 10/25/2021 SHE became ill around 3am and she fell. She was taken to hospital where she was placed in CDU bed 20 for 30 hours. She stated symptoms as paralysis from the neck down (no pain or sensation), No ability to speak/ slurred speech, swelling of her arm. She said she was not diagnosed with any other condition as providers only suspected the immunizations being the culprit. Patient states that the hospital healthcare providers told her she should never get two shots on the same day and it was potentially due to that.

I had her sit down and rest and gave her an ice pack for her back and after about 15 minutes she was feeling better and finished her shopping and went home. Patient received two vaccines, Shingrix and Pneumovax23 on 4/19/22. The shingles shot was given in the right arm and pneumonia in the left. She presented the day after administration barely able to move her left arm - the arm that the pneumonia vaccine was administered. She was stating that her left arm hurt really bad (in the shoulder joint) and upon checking, the area did look swollen and irritated. The location of the vaccine administration was higher than normal. I advised her to make an appointment with her primary care physician and if safe, take OTC anti-inflammatory medication for the pain. My impression was that the vaccine was administered too high, resulting in SIRVA (shoulder injury related to vaccine administration).

The immediate area around the patient's right arm injection site was "fine"/ patient experienced redness and swelling of the upper half of the right arm above the elbow; The immediate area around the patient's right arm injection site was "fine"/ patient experienced redness and swelling of the upper half of the right arm above the elbow; This spontaneous report has been received from a pharmacist referring to a 77-year-old female patient. The patient's medical history included breast cancer. The reporter stated that she believed the patient underwent a lumpectomy on the right breast. The patient had allergy to latex, sulfonamide (reported as "sulfa"), sulfamethoxazole (+) trimethoprim (BACTRIM) and penicillin. Her concomitant medications included levothyroxine sodium (SYNTHROID) and anastrozole. Historical vaccines included pneumococcal vaccine conj 7v (crm197) (PREVNAR). On 24-FEB-2022, the patient was vaccinated with pneumococcal vaccine, polyvalent (23-valent) (PNEUMOVAX 23) 0.5 ml one time administration, intramuscularly (IM) in her right arm (lot number U010534 has been verified to be valid, expiration date reported and established as 31-OCT-2022) for pneumonia prevention. On 24-FEB-2022, the patient experienced redness (erythema) and swelling of the upper half of the right arm above the elbow (peripheral swelling). The reporter stated that the immediate area around the patient's right arm injection site was "fine". On 26-FEB-2022, the patient told the reporter that the redness and swelling was better. On 28-FEB-2022, the patient told the reporter that the redness and swelling was almost gone. It was reported that the patient did not seek medical attention, and no treatment was given for the events. Additionally, it was reported that the patient did not have reactions to any other vaccines. At the time of reporting, the patient was recovering from erythema and peripheral swelling. The causal relationship between pneumococcal vaccine, polyvalent (23-valent) (PNEUMOVAX 23) and the events was not provided.

On 2/27/22 patient had 2 vaccines administered (Shingrix & Pneumovax 23) and today (3/3/22) came in with a red rash at the injection sites and reported it was hot and itchy. She stated it started shortly after she received the vaccines. She did say the itch and hot had cleared up though and it was doing better. She is going to show her doctor tomorrow. Rph also recommended over the counter treatment.

Pt walked into facility and requested to be seen per having pain, redness and swelling of her left upper arm. Per patient she received the Pneumovax 23 vaccine on Monday, 2/28/22 during a visit with her provider. Patient states that upon returning home that day her arm was very red and painful. Patient c/o pain and stiffness in her neck. Observed large red area on skin of upper left arm approximately 5.5 x 5 inches of which patient identified was around and below the injection site of the vaccine. Area was hot to touch with some puffiness, and painful to touch. Per patient the redness was improved from Monday evening and yesterday. Patient denied and history of alllergies/difficulties after receiving any other vaccines.

Within two hours of receiving the vaccine, the patient experienced burning pain and weakness in both hands and then arms. Later that evening she felt a similar pain in her groin. She found that she could not use her hands to perform basic activities such as wiping herself after toileting or carrying household objects because her hands were so weak. She ended up going to the Emergency Department because she was so scared about the severity of pain and weakness in her hands, arms, and groin/upper thighs. This has slowly gotten better over the past week and she is able to function again. But she does not feel she is back to her previous baseline.
