No additional AEs; expired dose of VAQTA was administered; the vaccine had been involved in a previous temperature excursion; This spontaneous report has been received from a licensed practical nurse (LPN) referring to a patient of unknown age and gender. The patient's concomitant medication, pertinent medical history, drug reactions or allergies were not provided. On 14-FEB-2023, the patient was vaccinated with an expired dose of hepatitis A vaccine, inactivated (VAQTA) vial 50 (units not provided) (1 milliliter, lot # U013476 has been verified to be valid, expiration date was reported and established as 06-FEB-2023 upon internal validation) intramuscularly for prophylaxis (expired product administered). It was also reported the vaccine had been involved in a previous temperature excursion (product storage error). The maximum storage temperature of the vaccine was 49.82 degrees Fahrenheit for a total time frame of 1 hour 30 minutes. No other information was provided. No additional adverse event (AE) or product quality complaint (PQC) was reported.

On an unknown date, the patient was vaccinated with an adult dose of hepatitis a vaccine, inactivated (VAQTA) intramuscular injection, lot #U013476, expiration date: 06-FEB-2022, administered for prophylaxis (strength, exact dose and route of administration were not provided) (product administered to patient of inappropriate age).
