Symptomtext

No additional AEs were reported; A nurse reported that a 14 year old patient inadvertently received PEDVAX HIB on 7/18/2022; This spontaneous report has been received from a nurse referring to a 14-year-old patient of unknown gender. Their pertinent medical history, concurrent conditions, concomitant medications and drug reactions or allergies were not reported. On 18-JUL-2022, the patient was vaccinated with a dose of Haemophilus b Conjugate Vaccine (Meningococcal Protein Conjugate) (LIQUID PEDVAXHIB) as prophylaxis (dose regimen and route of administration were not provided). Lot# U014484, expiration date; 15-SEP-2023. The nurse reported that the patient received inadvertently a dose of Haemophilus b Conjugate Vaccine (Meningococcal Protein Conjugate) (LIQUID PEDVAXHIB) (Product administered to patient of inappropriate age).

Symptomtext

Infant appeared to tolerate vaccines well. At approximately 2100, Dad was reportedly carrying infant, infant had an episode of limpness, pallor, perioral cyanosis, improved with Dad positioning neck. EMS was called and arrived, infant was pale. Evaluated in ED, hospitalized for approximately 1.5 days for observation/testing, no clear cause found for event.
