Symptomtext

Patient developed large area of redness around what appears to be the injection location of either the MMR or the Hep A injections. Reaction is on the left otter/posterior leg above the knee. Although documented that 1 vaccine was on the left thigh and one on the right. Both injections sites appear to be on the left outer leg based on this reaction and another location of what appears to be a needle stick and bruise. This is a situation of an improperly placed injection and a localized site reaction.

She received her immunizations earlier today (DTaP-IPV, MMRV and Influenza) at about 11 am this morning. She went home and had lunch. After eating lunch, she complained that her stomach hurt. She had complained of stomach pain after breakfast also. Mom told her to go use the bathroom to see if that would help with her stomach ache. She voided and passed a BM. Mom wiped her afterwards and noticed red "discharge" on the toilet paper. Mom then had her lay down so she could see where the blood was coming from. She wiped her bottom and there was no blood so the pink discoloration was not coming from her anus. The urine in the toilet bowl was clear yellow, not pink tinged. When asked if she had pain with urination, pt. responded yes. She had not stated anything about painful urination prior to being asked. ROS neg fever, back pain, vomiting, enuresis, urinary urgency or urinary frequency. No recent bubble baths

No additional AE; Medical Assistant calling to report that a 4-year old female patient was accidentally administered an extra dose of PROQUAD on July 8, 2022. Patient had previously finished the vaccination series on January 11, 2022. No additional AE information prov; This spontaneous report has been received from a medical assistant, concerning a 4-year-old female patient. The patient's medical history, concurrent conditions, previous drug reactions, allergies, and concomitant therapies were not provided. On an unknown date, the patient was vaccinated with measles, mumps, rubella and varicella (Oka-Merck) virus vaccine live (PROQUAD) injection, as prophylaxis (dose, route of administration, anatomical location, site of injection, lot number and expiration date were not provided). On 11-JAN-2022, she finished the vaccination series. On 08-JUL-2022, the patient was accidentally administered an extra dose of measles, mumps, rubella and varicella (Oka-Merck) virus vaccine live (rHA) (PROQUAD) injection, at a dose of 0.5 milliliters (ml), lot number U017137 had been verified to be a valid number, expiration date reported and validated as 28-NOV-2022 , and upon internal validation established as the same date (route of administration and anatomical site of injection were not provided) as prophylaxis (extra dose administered). No additional adverse events were reported in the patient.

No additional AE information provided.; Customer report that PROQUAD LOT U017137 expiration date 11/28/2022 was administered to a 4-year-old patient on 31-MAY-2022 after a temperature excursion; This spontaneous report was received from a registered nurse and refers to a 4-year-old male patient. No information regarding the patient's concurrent conditions, medical history and concomitant medications was provided. On 31-MAY-2022, the patient was vaccinated with improperly stored (product storage error) dose of measles, mumps, rubella and varicella (oka-merck) virus vaccine live (recombinant Human albumin) (PROQUAD) lot #: U017137, expiration date: 28-NOV-2022, administered subcutaneously at left leg (exact dose and strength was not provided) for prophylaxis. Temperature excursion information: due to loss of electrical power to freezer where vaccine was stored, temperature went out of suggested manufacturer range of -30 degree Celsius to -15 degree Celsius, with warmest reading of -3.6 degree Celsius for a duration of 40 minutes (reported also as: -14 degree Celsius up to -10 degree Celsius for 50 minutes; -10 degree Celsius up to 8 degree Celsius for 10 minutes). Temperature excursion occurred as a result of a reading from a digital data logger. The vaccine did not experience any prior temperature excursions. There was no additional adverse event reported The event of improper storage of product in use was considered as an important medical event by a reporter.

Symptomtext

Patient given a Pentacel at her well child exam which included an additional HIB vaccine that was not needed in series. This was reported immediately to her physician. Dr. stated this was not worrisome or concerning whatsoever. Vaccine coordinator was notified immediately as well as family.
