Symptomtext

Patient developed a nontender erythematous patch at the site of the injection, about 9-10 days after it was given. This patch coalesced into several red NT papules, and parent described a vesicular lesion which was no longer present at the day of the follow up visit.

no other adverse events reported; given an expired VARIVAX; temperature excursion/Temperature 46F; This spontaneous report was received from a medical assistant referring to a patient of unknown age and gender. The patient's concomitant medication, pertinent medical history and drug reactions/allergies were not reported. On 18-JUL-2022, the patient was vaccinated with one dose expired varicella virus vaccine live (oka/merck) (VARIVAX) subcutaneous injection (lot #U020239 had verified to be a valid lot #, labeled expiration date: 28-JUN-2023; route, strength and anatomical location was not provided) for prophylaxis (expired product administered). The vaccines varicella virus vaccine live (oka/merck) (VARIVAX) underwent temperature excursions of 46 Fahrenheit (F) for 264 hours (product storage error). The expiration date was short dated to 11-MAY-2022 after experiencing a temperature excursion. It was reported that the suspect vaccine was administered after short-dated expiry date but prior to labeled expiry date. There was no previous temperature excursion. And the reporter inquired about revaccination. No additional adverse event details were provided (no adverse event).

No other AE ocurred; Administered to a patient after a temperature excursion; This spontaneous report was received from an office manager concerning a patient of unknown age and gender. Patient's pertinent medical history, drug reactions or allergies and concomitant medications were not provided. On 07-MAR-2022, in March 2022, the patient was vaccinated with measles, mumps, and rubella (wistar ra 27-3) virus vaccine, live recombinant human albumin (rHA) (M-M-R II) 1 dose (lot number T033815 with expiration date on 05-OCT-2022) and with varicella virus vaccine live (oka/merck) (VARIVAX) 1 dose (lot number U020239 with expiration date on 28-JUN-2023) both for prophylaxis (route of administration, vaccination scheme frequency and anatomical location were not provided). Before administration, the doses of the vaccines experienced a temperature excursion on 07-MAR-2022 since they were stored at a temperature of -14 degrees Celsius (C) up to -10 degrees C for the time frame of 30 minutes, from -10 degrees C up to 8 degrees C for 2 hours and 15 minutes, and from 9 degrees C up to 25 degrees C for 20 minutes (product storage error). There was no previous temperature excursion. No adverse events were reported.

Vaccines given despite temp excursion.; This spontaneous report was received from a licensed practical nurse (LPN) regarding to a 15-year-old female patient. The patient's concurrent conditions, pertinent medical history, concomitant therapies, drug reactions, and allergies were not provided. On 28-JAN-2022, the patient was vaccinated with an improperly stored doses HPV rL1 6 11 16 18 31 33 45 52 58 VLP vaccine (yeast) (GARDASIL 9) lot number U007667 with an expiration date of 23-MAY-2023 second dose, intramuscularly, in her right deltoid (dose, strength and formulation were not reported) and varicella virus vaccine live (Oka/Merck) (VARIVAX), lot number: U020239, expiry date: reported as 23-JUN-2023, but upon internal validation, established as 28-JUN-2023, second dose, subcutaneously, in her left arm (strength, dose and formulation, were not reported) for prophylaxis (product storage error). There were no known patient's adverse events (no adverse event).
