>>MESSAGE (06/30/22 09:50 AM): >> Mom called and stated pt. received vaccines last week. He developed a fever on Tuesday. Tmax 104.4, but stays around 100-101 with motrin and tylenol. She was wondering whether the MMR would cause this. I spoke w/ Dr. and reviewed this online and per website "around 7 to 11 days after the injection, babies or young children may feel a bit unwell or develop a high temperature for about 2 or 3 days." Dr. recommended a covid test to be sure and to monitor for other symptoms in case it is not a vaccine reaction. Relayed this to mom and advised her to call us tomorrow if the fever has not broken. I spoke w/ mom on 7/1/22 and she stated pt.'s fever has come down to 100.3 without Tylenol or Motrin. Advised to continue to monitor. If his fever resumes or he begins not feeling well over the weekend, advised to contact our after-hours line for further instruction.

No side effects were reported; HCP calling to report that expired MMRII was inadvertently administered to a patient today, 11/1/2023. HCP reports the vaccine had not experienced any temperature excursions. No side effects were reported. No additional information/clarification prov; This spontaneous report was received by a pharmacist reporting a patient with 49 years old of unknown gender. The patient's pertinent medical history, concurrent conditions and concomitant therapies were unknown by the reporter. Health Care professional (HCP) was calling to report that expired Measles, Mumps, and Rubella (Wistar RA 27-3) Virus Vaccine, Live (M-M-R II)albumin status rHA (route was not reported) (0.5 ml, lot# U022174, expiration date: 14-JUL-2023 for prophylaxis) was inadvertently administered to this patient today, (01-NOV-2023). Additionally patient received sterile diluent (MERCK STERILE DILUENT) 0.5 ml. HCP reported the vaccine had not experienced any temperature excursions. No side effects were reported. No additional information/clarification provided. No additional Adverse Event (AE)/no Product Quality Check (PQC).

HCP did confirm there was no T/E for this product. Permission was granted to contact HCP. No additional AE reported; This spontaneous report was received from a nurse referring to a patient of unknown age and unknown gender. The patient's concurrent conditions, pertinent medical history, drug reactions/allergies and concomitant medications were not reported. On 25-JUL-2023, the patient was vaccinated with an expired dose of measles, mumps, and rubella (wistar ra 27-3) virus vaccine, live (rHA) (M-M-R II) subcutaneous injection (lot #U022174, expiration date: 14-JUL-2023; strength was not provided) administered subcutaneously as prophylaxis (expired product administered). It was confirmed there was no temperature excursions (T/E) for the vaccine. There was no additional adverse event (AE) reported.

**3:30 pm incident report: after vaccines where injected stepmother told MA that patient just recieved the MMR this morning from pharmacy. She handed her a copy of the paper. It was a form having pharmacy logo and MMR administered on 8/9/22. MOP apologized for not informing me nor corrected me when I explained to her and FOP that the patient needs catch-up for MMR (. I even went slow in saying MMR w/c is Measles, Mumps, Rubella combined with Varicella.) Varicella, DTaP and Polio, based on documentation as of 3 pm) information. FOP and stepmother agreed with all vaccines advised & offered. Upon being informed by my MA of error, I discussed with MOP possible side effects. Will submit VAERS and report accidental receipt of MMR vaccine to Merck today and determine any more side effects known to them. Advised MOP to submit VAERs as well, reporting this incident.
