Patient reported to have seizure episode for 10 minutes. Seen at ED and then transferred to hospital and had CT, LP , labs and EEG completed. Required versed to resolve seizure episode. Since then has been evaluated in ED and hospital due to multiple episodes of seizure activity.

Symptomtext

Patient received 30 ug of Pfizer bivalent vaccine rather than 3 ug. Multidose vial used with 10 doses however no dilution used and so patient received entire content of vial. Contacted the family later in the day when it was realized patient received incorrect dose to let them know of the error. Discussed starting Tylenol every 6 hours and to monitor for symptoms and to use ibuprofen for breakthrough fevers or pain. Reviewed if pain or fevers not resolving with medication or if any other concerning symptoms such as lethargy, inability to walk, poor tone needs urgent evaluation. Patient was asymptomatic at that time. Attempted to call family on 4/22 but no answer, left voicemail. Spoke to mother at 8:30 am on 4/24. Mother reported that patient had fevers over prior 2 days although did improve with Tylenol and Motrin. Did report that patient was having trouble walking however but she thought it might be related to pain. Patient not yet awake and so mother unaware of how he was doing at time of report. Let her know to seek evaluation in ER if unable to walk.

Symptomtext

Patient came to office for 3 month exam. Received all of her 3 month. Per provider mom requested flu vaccine as well. Flu vaccine administered into left thigh and tolerated it well. Pt remained in the office for 10 or so minutes following vaccination and showed no s/s of reaction. Mom notified that vaccine was given at 3 months instead of 6.

Patient was given Pfizer 5-<12 and should have been given Pfizer 6 months-4 years. Parent informed at appointment. Patient tolerated all vaccines given. This nurse has not had any other further conversation with the parent at the time of completing this form.

No additional AE reported; PEDVAX HIB was administered after temperature excursion/Temperature 32.79F for 15 minutes; 34.22F for 15 min; This spontaneous report was received from a licensed visiting nurse (L.V.N), concerning a patient of unspecified age, gender and race. The patient's medical history, concurrent conditions and concomitant therapies were not reported. On 20-APR-2022, the patient was vaccinated with haemophilus b conjugate vaccine (meningococcal protein conjugate) (LIQUID PEDVAXHIB), 0.5 mL (lot # U024089 has been verified to be a valid lot # for haemophilus b conjugate vaccine (meningococcal protein conjugate) (LIQUID PEDVAXHIB), expiration date: 27-OCT-2023) (strength, route of administration, and anatomical location were not provided) as prophylaxis. It was reported that the vaccine underwent a temperature excursion of 32.79 F degrees and 34.22 F degrees, each for a duration of 15 minutes (Product storage error). It was also reported that no previous excursions had presented. There were no further adverse events or additional information available.

Concomitant medication, pertinent medical history and drug reaction/allergies were not reported. On 21-FEB-2022, the patient was vaccinated with improperly stored haemophilus b conjugate vaccine (meningococcal protein conjugate) (LIQUID PEDVAXHIB) (strength, dose and route were not reported; lot # U024089, expiration date: 27-OCT-2023) for prophylaxis (product storage error). It was reported that the suspect vaccine was exposed to a temperature of minus 59.0 Fahrenheit (F) for a time frame of 2 hours and 47 minutes (the temperature excursion occurred on 19-FEB-2022). There were no previous temperature excursions. No additional adverse event (AE) information was provided (no adverse event).

On 15-FEB-2022, the patient was vaccinated with an improperly stored dose of rotavirus vaccine, live, oral, pentavalent (ROTATEQ) oral liquid, 2 mL one time, lot #1742464, expiration date: 20-SEP-2022, administered orally for prophylaxis (strength was not provided); and haemophilus b conjugate vaccine (meningococcal protein conjugate) (LIQUID PEDVAXHIB) intramuscular injection, 0.5 mL one time, lot #U024089, expiration date: 27-OCT-2023, administered for prophylaxis (strength, and route of administration were not provided). The administered doses of the vaccines experienced temperature excursions in the range above 47?F (47.5?F) for 20 minutes; above 47?F (46.7?F) for 10 minutes; and above 4 7?F (47.1?F) for 20 minutes. Total time frame: 50 minutes. The vaccines had not had any previous temperature excursions. It was not reported if it was recorded by a data logger. There was no adverse effect reported.
