The patient's medical history, concurrent conditions, concomitant therapies and previous drugs reaction or allergies were not reported. On 17-JAN-2023, the patient was vaccinated with a pediatric dose of hepatitis a vaccine, inactivated (VAQTA), 0.5ml, lot #U024094 that upon internal review was found valid as vial formulation, expiration date: 10-MAY-2023, administered for prophylaxis (strength, and route of administration were not provided (Product administered to patient of inappropriate age). No additional adverse event was reported.

The patient's medical history, concurrent conditions, and concomitant therapies were not reported. On 22-SEP-2022, the patient was inadvertently vaccinated with hepatitis A vaccine, inactivated (VAQTA) 0.3 milliliter, lot #U024094, expiration date: 10-MAY-2023 (strength, anatomical location, and route of administration were not provided) as prophylaxis, instead of a COVID-19 vaccine (manufacturer unknown) (wrong vaccine administered). The reporter stated that the patient received a partial dose of hepatitis A vaccine, inactivated (VAQTA). No adverse event was reported.

Patient was administered VAQTA after temperature excursion on 05-JAN-2022.; This spontaneous report was received from a medical assistant regarding a patient of unknown age and gender. The patient's concurrent conditions, pertinent medical history, concomitant therapies, drug reactions, and allergies were not provided by the reporter. On 29-NOV-2021, Hepatitis A Vaccine, Inactivated (VAQTA) experienced a temperature excursion. Therefore, on 05-JAN-2022, the patient was vaccinated with a improperly stored dose of Hepatitis A Vaccine, Inactivated (VAQTA) (incorrect product storage) lot number U024094, expiration date 10-MAY-2023 (dose and route of administration were not provided). Symptomtext

Event: Pediatric Pfizer Vaccine Lot Number FK5127, was improperly stored in an Immunizations Freezer that maintained a temperature between -15 Celsius and -24 Celsius. Per the Manufacture?s guidelines the vaccine must be stored in a Refrigerator maintaining temperature of 2-8 Degrees Celsius or an Ultra-Low Cold Freezer maintaining temperature of -90C or lower. The patient received the vaccine that was improperly stored. Treatment: Not required, patient did not report any adverse reactions to the Vaccine. Outcome: Patient?s guardian notified and offered to restart the Pediatric Pfizer Covid Vaccine series again stored at the proper temperature. Signs, Symptoms course, etc. None reported by the patient?s guardian
