Symptomtext

Pt developed lump in bilateral thighs. Was not immediately reported by parents, unknown when lumps developed. Pt seen in office 6/15/22 for deceleration in weight gain. 6/27/22 pt started on iron supplement for iron deficiency anemia. Pt seen again 7/7/22 for mass of both thighs and congestion. US ordered and to be done when family returns back from vacation over this next weekend. Pt seen 7/20/22 for abscess. Mother notes that the left side area opened up and began draining purulent material, and mother notes that it is now significantly decreased in size. Pt developed COVID 19 and subsequently ultrasound not completed until 7/28/22.

ultrasound 7/28/22 CLINICAL INDICATIONS: Right and left anterior thigh lumps. COMPARISON: No prior exams are available for comparison. FINDINGS: Scanning over the right anterior thigh lumps reveal multiple complex fluid collections, including a 2.1 x 1.9 x 1.2 cm collection and a 2.0 x 1.2 x 0.8 cm collection in the subcutaneous fat. Color Doppler analysis reveals increased vascularity at the periphery of these fluid collections. There is also a borderline enlarged 18 x 8 x 5 mm lymph node. There is a 4.0 x 3.0 x 1.1 cm complex fluid collection in the anterior left thigh subcutaneous fat with peripheral vascularity on color Doppler analysis. IMPRESSION: There are 2 complex fluid collections in the anterior right thigh subcutaneous fat measuring 2.1 and 2.0 cm which may represent hematomas or small abscesses. 4.0 cm anterior left thigh subcutaneous complex fluid collection is also consistent with abscess or hematoma

18 month old patient presented on 3/30 for well-visit and was prescribed to HiB PRP-OMP conjugate vaccine 3 dose IM (PedvaxHIB) and MMR and Varicella combined vaccine SQ (PROQUAD), age 12mo?12yo by MD. CMA prepared vaccines and administered Hib (PRP-OMP, PedvaxHIB) IM in patient's left thigh (LOT # U024489) and MMRV subcutaneously in patient's left thigh (LOT # U029322). On 4/5, parent sent message noting patient had "knot at injection site". RN responded on 4/6 asking following up questions. RN also noted that following MMRV vaccine is common to see a rash develop and low grade fever (up to 7-10days post injection). CMA prepared vaccines and administered Hib (PRP-OMP, PedvaxHIB) IM in patient's right thigh (LOT # U024489) and Hepatitis A, IM in patient's right thigh (LOT # 95EJ7). CMA documented "pt tolerated well". On 4/2, parent sent message noting patient had fever (102) day after injections and still in pain. RN responded noting fever post injection indicated muscle administration and immune response triggered. System lists the following as adverse reactions for PedvaxHIB include irritability, pain, swelling, tenderness at injection sites, and fever. Patient's symptoms are likely and not unusual reaction for vaccine administration. Possible contributing cause could be if vaccine given in suboptimal location (preferred upper, lateral thigh)--same CMA administered vaccines. As patient was receiving only two injections, unclear why both were given in the same thigh.

6 month old infant presented on 3/30 for well-visit and was prescribed to receive DTaP HepB IPV combined vaccine IM (PEDIARIX), HiB PRP-OMP conjugate vaccine 3 dose IM (PedvaxHIB), Pneumococcal conjugate vaccine 13-valent IM (PREVNAR 13), and Rotavirus vaccine pentavalent 3 dose oral (ROTATEQ) by MD. CMA prepared vaccines and administered DtaP/HepB/IPV IM in patient's right thigh (LOT# 552F4), Hib (PRP-OMP, PedvaxHIB) IM in patient's left thigh (LOT # U024489), Pneumococcal conjugate 13-valent (Prevnar 13) IM in patient's left thigh (LOT # EX1981) and Rotavirus Pentavalent orally (LOT # U006165). CMA documented "pt tolerated well". Medical Assistant reporting the administration of PEDVAX HIB to a 12 year old Patient.; No additional adverse events reported.; This spontaneous report was received from a medical assistant concerning a 12-year-old patient of unknown gender. Information regarding the patient's concomitant therapies, current conditions, medical history and historical drug was not provided. On 31-MAR-2022, the patient was vaccinated with Haemophilus b Conjugate Vaccine (Meningococcal Protein Conjugate) (LIQUID PEDVAXHIB) 0.5 milliliters, lot number U024489 expiration date 02-NOV-2023 (route of administration were not provided) administered as prophylaxis (Product administered to patient of inappropriate age.)
