Symptomtext

Received MMR vaccine within about 15 minute started itching, difficulty swallowing, and rash. Had taken Benadryl and Pepcid about 15-30 minutes prior to receiving the vaccine. Then given Tagamet 400mg and 125mg of solu-medrol. These symptoms improved after treatment and then around 1:40pm numbness, tingling, burning, and tremors started. The numbness, tingling, and burning was from knees down.

Chest pain Dizziness Nausea

Symptomtext

5:50 PM after 15 min observation, pt left facility but was experiencing nausea and dizziness when she got to the car. Patient accompanied by sister (ROI and permission from mom on file). RN assessed patient and documented vital signs, WNL. Presented to Dr. Transported patient via wheelchair back inside clinic for further observation. Patient complained of left sided chest pain 10/10. Presented to Dr. Provider assessed patient and requested EMS. 2nd set of vital signs documented. Patient states intermittent chest pain. Mother arrived and EMS arrived. Dr. gave report to EMS. Mother and patient left via EMS transport to hospital.

Child has not been feeling well since 09/13. She continues to be achy all over, she is spiking random fevers. She has had diarrhea. After the vaccines almost immediately she spiked a fever and body aches, and GI issues. The fatigue and GI issues have persisted. She slept from 5pm to 11am the next day. This is unusual. Offered an office visit and mother declined, will wait another week to see if things get better. If not, mother will let office know and she will be scheduled ASAP

On 14-AUG-2023, the patient was inadvertently vaccinated with an expired dose of measles, mumps, and rubella (wistar ra 27-3) virus vaccine, live(M-M-R II) (dose: 0.5 ml, albumin status: recombinant Human albumin (rHA), lot # U025200, expiration date: 13-AUG-2023). Diluent with lot# W011291 and expiration date: 20-NOV-2024 was used. No additional adverse event (AE) was reported.

On 16-JUN-2022, the patient was vaccinated with improperly stored doses of measles, mumps, and rubella (wistar ra 27-3) virus vaccine, live (recombinant human albumin) (M-M-R II) subcutaneous injection, 0.5 milliliter, lot #U025200 was confirmed to be valid, expiration date: 13-AUG-2023, administered subcutaneously (reported as "SQ") (strength and site of administration were not provided) and hepatitis a vaccine, inactivated (VAQTA) intramuscular injection, 0.5 milliliter, lot #U031911 was confirmed to be valid, expiration date: 11-NOV-2022, administered intramuscularly (reported as "IM") both for prophylaxis (strength and site of administration were not provided). Both vaccines were exposed to the temperature of 13.5 degrees Celsius for 10 minutes (product storage error). No previous temperature excursions were reported. No additional adverse events or product quality complaints were reported (no adverse event).
