Symptomtext

Patient had severe erythema and edema surrounding the injection site of the Kinrix vaccine starting 05/12/22 with some improvement as of the morning of 05/13/22. Patient was treated with cetirizine 5mg once daily and diphenhydramine 12.5mg every 6 hours. Recommended application of cool compresses to affected area throughout the day. Recommended ibuprofen every 6-8 hours as needed for pain. Patient also experienced multiple episodes of vomiting and low-grade fever. He had pain of the left shoulder and limited range of motion of the left arm. Parents advised to closely monitor patient and will return patient to office as needed for new or worsening symptoms. Outcome unknown as treatment began this morning.

Symptomtext

Had his immunizations on 8/17, and then on 8/20 woke up and had some right testicular discomfort. The pain worsened and they brought him into the ER and he was diagnosed with epididymitis. No evidence of UTI so treated conservatively and he is doing better now. I do not know if this is a side effect of the vaccine or unrelated. Also reports some leg pain and knee pain but it is migratory and bilateral and not affecting him playing. The DTaP IPV was his first dose of Kinrix, but he has had 4 previous doses of DTaP and 3 previous doses of IPV. We did an MMR V ProQuad combo and this is the first time he received the combination, previously had MMR and varicella separately.

No additional AE.; This spontaneous report was received from a nurse and refers to an unknown age and gender patient, whose medical history, concurrent conditions, and previous adverse drug reaction/allergies were not provided. On 29-JUN-2022, the patient was vaccinated with an improperly stored dose of measles, mumps, rubella, and varicella (oka-merck) virus vaccine live (recombinant human albumin, rHA) (PROQUAD) subcutaneous injection, administered for prophylaxis (lot number: U026799, has been verified to be a valid lot number, and expiration date: 26-FEB-2023; strength, exact dose, vaccination scheme frequency, route of administration, and anatomical location were not provided). The suspect vaccine experienced a temperature excursion of -9.3 degrees Celsius (?C) for a time frame of 27 minutes. No previous temperature excursion occurred. No additional adverse events were reported.

No additional AE reported; 23 year old patient was administered a dose of PROQUAD on 13-APR-2022; the patient should received MMR and Varicella separately and not the combined dose; This spontaneous report was received from a certified medical assistant and refers to a 23-year-old female patient. The patient's medical history and concurrent conditions were not reported. On an unknown date, the patient had viral titre measurement done, which showed that the patient was not fully immunized against measles, mumps, and rubella (MMR) and varicella. On 12-APR-2022, she received tuberculin PPD (purified protein derivative of tuberculin (TB-PPD)) as a part of PPD test. The patient's concomitant vaccination included unspecified hepatitis B (Hep B) adult vaccine (manufacturer reported as GlaxoSmithKline (GSK)). On 13-APR-2022, the patient was vaccinated by a healthcare professional with measles, mumps, rubella and varicella (oka-merck) virus vaccine live, recombinant human albumin (rHA) (PROQUAD) at a dose of 0.5 ml, subcutaneously (SQ) in the left arm, lot # U026799, expiration date 26-FEB-2023 to repeat the immunization series as required by her grad school (prophylaxis). The vaccine was reconstituted with a sterile diluent (exact volume of the diluent, lot number and expiration date were not reported). The reporter stated, that according to the patient's age, she should be vaccinated with MMR and varicella vaccine separately instead of the combined dose (product administered to patient of inappropriate age, wrong vaccine administered). No adverse events were reported (described also as the patient had no response and no reactions). No prescription drug treatment was required for the reported experience.

Symptomtext

Patient stated that she could not walk and began to lower herself to the floor slowly and then began clench her arms and hands and lay on the floor. She also lost control of her bladder during this episode. Provider was present for the end of the episode. After about 30 seconds the patient was able to sit and talk and respond accordingly, but was tearful.

Concomitant therapies included hepatitis b vaccine (recombinant) (manufacturer unknown), covid-19 vaccine, polio vaccine and influenza vaccine (FLU). On 16-DEC-2021, the patient was vaccinated with the second dose of measles, mumps, rubella and varicella (oka-merck) virus vaccine live (PROQUAD) (strength, dose, and route were unknown; recombinant Human albumin; lot # U026799 has been verified to be a valid lot number, expiration date not reported, but upon internal validation established as 26-FEB-2023) in the left arm for prophylaxis (product administered to patient of inappropriate age). The patient was observed for 30min and no adverse reactions noted.
