Symptomtext

Patient started with fussiness and a fever at home, rash began roughly 36 hours after dose given. Started flat and red then quickly progressed to vesicular and fluid filled, covering legs, groin, arms, some to trunk/neck, mom noted some in mouth. Patient seen in office on Friday (roughly 72 hours after immunization). PO intake improving, no more fevers, rash still present with primarily vesicles present, some starting to scab.

No additional AE details.; Inbound call from HCP who stated a 1 year old was inadvertently administered PROQUAD and VARIVAX concomitantl.y; This spontaneous report was received from a Registered Nurse regarding a 12-month-old patient, of an unknown gender. The patient's medical history, concurrent conditions, and concomitant medications were not provided. On 14-DEC-2022, the patient was inadvertently vaccinated with measles, mumps, rubella and varicella (oka-merck) virus vaccine live (rHA) (PROQUAD) lot #W010527, expiration date: 25-SEP-2023, (strength, dosage regimen, anatomical location, and route of administration were not provided) administered as prophylaxis; and varicella virus vaccine live (oka/merck) (VARIVAX) lot #U027326, expiration date: 02-SEP-2023, (strength, dosage regimen, anatomical location, and route of administration were not provided) administered as prophylaxis (Accidental overdose) which were reconstituted with sterile diluent, lot #W000452 and expiration date: 23-JUL-2024. No adverse events were reported.

Per caller, lot had short-dated expiration from 6/28/2023 to 10/12/2022 due to past temperature excursion; vaccine was administered on 10/25/2022. Information regarding the patient's medical history, concurrent conditions, concomitant therapies, and past drug allergies or drug reactions were not provided. On an unspecified date, the varicella virus vaccine live (oka/merck) (VARIVAX) lot# U027326 experienced an unreported temperature excursion and due to this the expiration date was short-dated expiration from 28-JUN-2022 to 12-OCT-2022 (product storage error). However on 25-OCT-2022, the patient was vaccinated with varicella virus vaccine live (oka/merck) (VARIVAX) subcutaneous injection, at a dose of 0.5 milliliters (ml) once administered for prophylaxis (lot #U027326 confirmed to be valid, expiration date reported as 12-OCT-2022, but upon internal validation the expiration date was assessed as 02-SEP-2023, conflicting information) (expired product administered and product storage error) (strength, and route of administration were not provided). The patient's medical history and concurrent conditions were not reported. Concomitant medications included sterile diluent. On 10-AUG-2022, the patient was accidently vaccinated with measles, mumps, rubella and varicella (oka-merck) virus vaccine live recombinant Human Albumin (PROQUAD) (rHA), 0.5 milliliter, lot #U029150, expiration date: 16-MAR-2023 (anatomical location was not provided) administered subcutaneously as prophylaxis, and varicella virus vaccine live (oka/merck) (VARIVAX), 0.5 milliliter, lot #U027326, expiration date: 02-SEP-2023 (anatomical location was not provided) administered subcutaneously as prophylaxis, instead of varicella virus vaccine live (oka/merck) (VARIVAX) and measles, mumps, and rubella (wistar ra 27-3) virus vaccine, live recombinant (M-M-R II), receiving and extra dose of varicella vaccine (accidental overdose and wrong product administered).
