Symptomtext

Allergic reaction to PROQUAD vaccine (rash to left arm), we gave Benadryl. child was observed for 30 min in office. lungs were clear she left with no distress/angioedema. Advise mother to go to ER or call EMS if distressed breathing or swelling to face; follow up in one day to assess reaction.

18 month old patient presented on 3/30 for well-visit and was prescribed to HiB PRP-OMP conjugate vaccine 3 dose IM (PedvaxHIB) and MMR and Varicella combined vaccine SQ (PROQUAD), age 12mo?12yo by MD. CMA prepared vaccines and administered Hib (PRP-OMP, PedvaxHIB) IM in patient's left thigh (LOT # U024489) and MMRV subcutaneously in patient's left thigh (LOT # U029322). On 4/5, parent sent message noting patient had "knot at injection site". RN responded on 4/6 asking following up questions. RN also noted that following MMRV vaccine is common to see a rash develop and low grade fever (up to 7-10days post injection). System lists the following as adverse reactions for PedvaxHIB include irritability, pain, swelling Additional information for Item 18:18 month old patient presented on 3/30 for well-visit and was prescribed to HiB PRP-OMP conjugate vaccine 3 dose IM (PedvaxHIB) and MMR and Varicella combined vaccine SQ (PROQUAD), age 12mo?12yo by MD. Patient's symptoms are likely and not unusual reaction for vaccine administration. Currently no follow up from parent to clarify follow up questions. As both PedvaxHIB and MMRV were given in same thigh, unclear as to which vaccine may have caused injection site reaction without further clarification from parent.

The patient's medical history, concurrent conditions and concomitant therapies were not reported. On 17-AUG-2022, the patient was vaccinated with measles, mumps, rubella and varicella (oka-merck) virus vaccine live (recombinant Human albumin) (PROQUAD) subcutaneous injection, lot #U029322, expiration date: 19-MAR-2023, administered for prophylaxis (anatomical location, dose, and route of administration were not provided) (Product administered to patient of inappropriate age). No additional AE reported; patient was accidently administered PROQUAD instead of MMR II; that patient was accidently administered PROQUAD at 36 years old; This spontaneous report has been received from a medical assistant to a 36 years old female patient. The patient's concurrent conditions, medical history, concomitant medications, previous drug reactions, or allergies were not provided. The patient had lab work done and her titers said that she needed measles, mumps, and rubella (wistar ra 27-3) virus vaccine, live (M-M-R II). On 31-MAY-2022, the patient was accidently administered measles, mumps, rubella and varicella (oka-merck) virus vaccine live (PROQUAD) (lot # U029322, expiration date 19-MAR-2023) instead (wrong vaccine administered and inappropriate age at vaccine administration). No additional adverse events (AE) reported.

Patient was supposed to receive MMR. Patient received MMRV, when staff realized that wrong vaccine was administered staff called parent to explain what happened and let mom know we will be monitoring the patient for the next 5 to 12 days. Patient at this time was asymptomatic.
