Fall Feeling hot Loss of consciousness

Symptomtext

Patient received MMR and Varivax vaccines. Patient was seated and asked for proof of vaccination. Pharmacist stepped away to grab and heard a thump. Ran out, patient was on the floor. Patient lost consciousness briefly (10 seconds) about 2 minutes after receiving vaccine and immediately after standing. Patient regained consciousness upon pharmacist speaking to her. Patient said she was warm, remained on floor and given water until EMS arrived. All vitals clear, EMS approved her to leave without being seen.

Syncope

Symptomtext

Child had an episode of syncope as he started to leave the vaccination area. He came to almost immediately. Subsequently given juice and placed in reclining chair. Mother stated that the child had not eaten breakfast yet. He was also given peanut butter crackers. We kept him additional 15 minutes after eating drinking. When child left he appeared to be fine.

Immediate post-injection reaction Presyncope

Symptomtext

The child is a under care. The child immediately had a vasovagal reaction after injection of HPV vaccine. She did not loss consciousness . The FNP was called into the room and had the child lie down and drink Gatorade and eat a popsicle. Less than 5 minutes after she started to eat the popsicle, color had returned to the child's face. Within 10 minutes she stated she felt better. She was able to walk normally and move all 4 extremities well. The child denied pain. No swelling or redness noted at the injection site. No respiratory distress noted. After the 15 minute observation period, the child left the office. The child and staff who had accompanied child were instructed to call the clinic as needed,

Symptomtext

Patient was immunized in Health Department and waited in treatment room to monitor for reaction prior to leaving. Patient denied any adverse reaction prior to leaving the health department. However, on her way home, approximately 20- 30 minutes post immunization, pt reported feeling as if her tongue and lips were swelling, along with generalized red, itchy hives. Pt took an over the counter Benadryl and sought immediate medical attention at the Hospital Emergency Department where she was given Intervenous Benadryl and a Steroid. Pt was then medically cleared and discharged home.

Symptomtext

Patient had to get vaccines 3 on left thigh and 3 on right thigh on 1/9/2023. Pt was getting 6 vaccines due to missing the 12 month vaccines. Had reaction on 1/10/2023 was seen again on 1/10/23. Reaction on both right and left thigh consisted of redness swelling and pain. Treatment on 1/10/2023 to give ibuprofen as needed and apply warm compresses. Mom called again on 1/11/20233 said reaction getting worse on both thighs. There is more redness and swelling. Provider ordered antibiotic and child will be seen again on 1/13/2023. Unknown which vaccines reacted due to patient having 6.

Symptomtext

Hep. B vaccine given in the left arm. MMR vaccine given in the right arm. Both were vaccines given to a healthcare provider following low titer labs. No prior vaccine records. Patient may have had MMR and/or Hep. B series in the past, but records not available. Patient developed hives on her face and neck about an hour after having the vaccinations. Associated with all over itchiness and chest tightness. She took some Benadryl with some improvement in symptoms and contacted the office by phone. She was referred to the ER for further evaluation and agreed to seek ER care.

Per the clinical lead the Flu and Hepatitis A vaccine was transported to clinic in the brown portable cooler. A vaccine excursion occurred with the brown portable cooler. The temperature on the data logger attached to the brown portable cooler went out of range according to Havrix and Flulaval manufacturer recommendations. The portable cooler was reconditioned and the Flu and Hepatitis A vaccine was administered to the patient. Manufacturer contacted by Health Department representative. Per the Health Department they were advised by GlaxoSmithKline that the Flu and Hepatitis A vaccine was not viable and the patient needed to be re-vaccinated.

The temperature on the portable cooler went out of range based on GlaxoSmithkline and Pfizer/Wyeth manufacturer recommendations. The portable cooler reconditioned and Pediarix vaccine and Prevnar 13 admistered to patient. Manufacturer contacted by representative. Per the Health Department they were advised by GlaxoSmithKline and Pfizer/wyeth that the Pediaxrix and Prevnar 13 vaccine was not viable and the patient needed to be revaccinated.

no adverse event; Caller reported vaccines administered after temperature excursion.; Caller also reported vaccines were administered IM.; This spontaneous report was received from a medical assistant and refers to the patient of unknown age and gender. There was no information about patient's concurrent conditions, concomitant therapies, or medical history provided. On 28-NOV-2022, the patient was vaccinated with dose of improperly stored measles, mumps, and rubella (wistar ra 27-3) virus vaccine, live (M-M-R II) subcutaneous injection, 0.5 ml, lot #U029619, and expiration date: 22-SEP-2023, albumin status has been verified as recombinant human albumin (rHA) (anatomical location was not provided); and dose of improperly stored varicella virus vaccine live (oka/merck) (VARIVAX) subcutaneous injection, 0.5 ml, lot #W005206, and expiration date: 14-FEB-2024 (anatomical location was not provided); both intramuscularly for prophylaxis (incorrect route of product administration). Administered doses of vaccines have been stored in the temperature of 58.5F for measles, mumps, and rubella (wistar ra 27-3) virus vaccine, live (M-M-R II) and 24.3F for varicella virus vaccine live (oka/merck) (VARIVAX) for 3 hours (product storage error). There was no previous temperature excursion reported.
