mild reaction 2 hours after vaccinations, mmr, varicella, and hep a given, areas of injection was red, hot and swollen. after 15 - 20 minutes are less redness and swelling advised to follow up 24 hrs. follow up next day and adverse reaction with swelling and redness in upper thigh are resolved.

Symptomtext

Mom reports the patient developed fevers with T-max 103F the day after she received her 12-month immunizations (MMR, varicella, and hepatitis A immunizations) on 11?29. She has had intermittent fevers daily but no fever noted today. She has been extremely fussy. She is consolable at times but has been crying on and off throughout the day and night. Parents noticed a faint pink rash on her trunk, chest, and extremities today. Parents deny any face/mouth swelling, difficulty breathing, vomiting, diarrhea, lethargy. Prescribed Tylenol and Motrin alternating every 3 hours to be taken at home after 4ml of Motrin was given in clinic.

Symptomtext

Patient developed intense pruritus over whole body. Few, diffuse hives noted on arm, back, neck. Redness and slight inflammation at sites where vaccines were given due to presence of bandaids. Patient developed these symptoms less than 24 hrs. of receiving the above listed vaccines. Patient stated that she did not have any adverse reaction to the first doses of these vaccines, nor has she ever had a reaction to a vaccine in the past. Patient has a stated allergy to sulfa drugs. Patient elected to see a physician today. She directly left Health office to be seen at clinic.

The subject's concurrent conditions, medical history and concomitant therapies were not reported. On 06-NOV-2023, the patient was vaccinated with a dose of measles, mumps, and rubella (wistar ra 27-3) virus vaccine, live (M-M-R II) rHA albumin status lot # U029620, the expiration date: 22-SEP-2023 (expired product administered), 0.5 milliliter, (route was not reported) for prophylaxis. Patient was given a Havrix, manufactured by GlaxoSmithKline, on 6/20/2022. The expiration date for this vaccine was 6/18/2022. Patient is not having any adverse reaction or symptoms. I am required to report this immunization error. GSK and CDC were contacted to confirm that patient's health is not in any danger. This was confirmed and GSK stated the potency/viability for this vaccine extends up to 30 days past expiration date. No further action needed.
