Symptomtext

Certified medical assistant was getting vaccines ready and she accidentally drew up a ProQuad instead of an MMR. Patient tolerated vaccines well, and parent was informed of what to watch for: Fevers/ febrile seizures/ and rash around the injection site.

Caller reported AE for expired Measles, Mumps, Rubella and Varicella, Virus Vaccine Live (PROQUAD) that was inadvertently administered to a patient.; No symptoms reported.; This spontaneous report was received from a medical assistant concerning a patient of unspecified age and gender. The patient's pertinent medical history, concurrent conditions, drugs reactions or allergies and concomitant medications were (unknown or not provided) by reporter. On 01-AUG-2023, the patient was invertedly vaccinated with an expired measles, mumps, rubella and varicella (oka-merck) (PROQUAD), subcutaneous injection, albumin status recombinant human albumin (rHA), 0.5 milliliter, administered for prophylaxis (Lot No. U031562 has been verified to be a valid lot number, expiration date reported as 06-APR-2023 and confirmed upon internal validation) (strength, and route of administration were not provided) (Expired product administered). Client was given MMRV instead of MMR vaccine along with Varicella vaccine for One year old shots. Writer had mistaken MMRV for MMR vaccine and grabbed wrong vaccine out of the freezer. Client's mother was contacted and explained the situation and educated to monitor client for adverse reactions or strong immune response.

The patient's medical history, concurrent conditions, and previous drug reactions or allergies were not reported. The patient's concomitant medications included sterile diluent. On 08-SEP-2022, the patient was vaccinated with 1 dose of measles, mumps, rubella and varicella (oka-merck) virus vaccine live rHA (recombinant human albumin) (PROQUAD) subcutaneous injection (lot number U031562 was verified to be a valid number, expiration date reported and established as 06-APR-2023), administered via intramuscular instead of subcutaneous for prophylaxis (exact dose, strength, anatomical injection site, and vaccination scheme were not provided) (incorrect route of product administration). No additional adverse events were reported.

Patient received extra dose of Varicella vaccine accidently. Mother made aware through phone call by Provider and Nursing Director. Per mother child is stable, no changes, mother reports no symptoms at this time. Advised to give Tylenol PRN and monitor patient for any changes, if changes occur take child to Emergency room. RTC for Follow up.
