Symptomtext

Mother elected to have child receive 4 vaccinations. 2 given in each arm. Two nurses injected the vaccines simultaneously. After the 2nd injection given - child immediately noted to have eyes roll back in head and slump in chair. Child regained conciousness after approx 10 seconds. Reported a headache. Child given water and fruit snacks. Said she was feeling better. HR 76, O2 98%. Child assisted back to lobby where family was. Child appeared pale, reported feeling dizzy and blurred vision. BP 80s/40s. Assisted to laying position on ground. Given more water. Cool packs place on forehead, wrists, and stomach. Encouraged to drink more water. Remained concious. BP rechecked after 10 minutes: 94/61 laying. HR 62. O2 96%. Face pink. Child reports feeling better. Approx 30 min after syncopal episode, assisted back to sitting. Recheck BP 101/64. Child able to ambulate without symptoms. Child left the office with mother.

Patient fainted falling forward. Team went in and helped her lay down, took vital signs. Patient was stated she was very dizzy, waited in the room until dizziness went away

Pt was given vaccine in Room. Stated she was alright left Room went to Reception. This nurse saw her in Reception asked if she was alright, she said she could use a juice. When this Nurse Returned with juice pt. had loss consciousness. Called code, called MD. MD and Nurse lifted pt to ground.

Patient received Gardasil 9 on 19-SEP-2022 and then had an allergic reaction; Rash at the injection site; This spontaneous report was received from Company Representative and refers to a 37-year-old female patient. No information regarding the patient's medical history, concurrent conditions, previous drug reactions or allergies and concomitant medications was provided. On 19-SEP-2022, the patient was vaccinated with hpv rl1 6 11 16 18 31 33 45 52 58 vlp vaccine (yeast) (GARDASIL 9) injection, lot #U037064, expiration date: 10-AUG-2024 (who had been verified to be a valid number), administered for prophylaxis on the right arm (strength and dose were not provided). On 24-SEP-2022 (reported as 5 days later), the patient had an allergic reaction, the reaction consisted of swollen lips and eyes as well as a rash at the injection site (hypersensitivity-injection site rash). The outcome of the adverse events as well as the causality assessment with hpv rl1 6 11 16 18 31 33 45 52 58 vlp vaccine (yeast) (GARDASIL 9) were not provided.

On 17-AUG-2022, the patient received the first dose of hpv rl1 6 11 16 18 31 33 45 52 58 vlp vaccine (yeast) (GARDASIL 9) injection (lot #U037064 has been verified to be a valid lot number for hpv rl1 6 11 16 18 31 33 45 52 58 vlp vaccine (yeast), expiration date not reported, but upon internal validation established as: 10-AUG-2024) administered as prophylaxis (strength, dose and route of administration were not provided). On 21-OCT-2022, the patient received the second dose of hpv rl1 6 11 16 18 31 33 45 52 58 vlp vaccine (yeast) (GARDASIL 9). On an unknown date after the second dose of the vaccine, the patient the patient had irritation, pain, swelling and redness at the injection site and eventually developed a "ping pong ball" size lump on the left shoulder. The outcome of the events, the action taken with the therapy and the causality assessment were not provided.

His concomitant therapies, pertinent medical history, and drug reactions/allergies were not reported. On 24-OCT-2022, the patient was supposed to receive a flu shot. However, he was inadvertently administered with HPV rL1 6 11 16 18 31 33 45 52 58 VLP vaccine (yeast) (GARDASIL 9) injection instead (strength, dose, route of administration were not reported; lot#: U037064, expiration date: 10-AUG-2024) for prophylaxis (product administered to patient of inappropriate age; wrong product administered). No additional adverse event (AE) was reported.

Symptomtext

Patient received vaccine on 8/23/22. On 8/25/22 patient developed changes in vision including a flash in their left lateral eye worsening to area of vision being slightly off center and a gray/white flashing spot that is like a curtain over field of vision. Patient had an emergent office visit and was initially diagnosed with ocular migraine related to SSRI they are taking. Patient was then seen by a retina specialist and was diagnosed with Multiple Evanescent White Dot Syndrome.
