4hr after vaccine was administered left eye started to swell and then went down. Right eye swelled, rash on buttocks and a few hives on body.

Symptomtext

Patient came in for a well child check. Two vaccines were ordered influenza and hepatitis A, vaccines were verified with provider. Vaccines were administered subcutaneously instead of intramuscularly. Patient had immediate swelling and redness, developed pain and limping a few hours later. Patient followed up with primary care 5/17 (a few days later) was still limping and complaining of pain and fever. Provider recommended supportive care.

No additional AE details were provided and no additional AE's were reported; patients that were administered vaccines after they were involved in a TE.; This spontaneous report was received from a licensed practical nurse and referred to a 1-year-old patient of unspecified gender. The patient's medical history, concurrent conditions, concomitant therapies were not reported. On 07-NOV-2022 at 11:10, the patient was vaccinated with hepatitis a vaccine, inactivated (VAQTA), 0.5 milliliter intramuscular injection, 0.5 milliliter, lot #U038841, expiration date: 06-MAR-2023, administered intramuscularly for prophylaxis in left thigh after the vaccine was involved in a temperature excursion (Product storage error). The storage temperature of the hepatitis a vaccine, inactivated (VAQTA) was 11.4 degrees Celsius for a time frame of 1 hour 30 minutes. There was no previous temperature excursion. No additional AE reported.; Caller is a medical assistant calling to report an adverse event for M-M-R II, VAQTA, and VARIVAX. Caller reports that the patient was administered all three vaccines on 13JUL2022, after they were exposed to a temperautre excursion.; This spontaneous report has been received from a medical assistant and refers to a patient of unknown gender and age. The patient's pertinent medical history, concurrent conditions, concomitant medications, and previous drug reactions or allergies were not reported. On 13-JUL-2022, the patient was vaccinated with improperly storage dose of measles, mumps, and rubella (Wistar RA 27-3) Virus Vaccine, Live (rHA) (M-M-R II) injection (lot number U022983 which has been verified to be valid, expiration date reported and validated as 22-JUL-2023), hepatitis A vaccine, inactivated (VAQTA) injection (lot number U038841 which has been verified to be valid, expiration date reported and validated as 06-MAR-2023), and varicella virus vaccine live (Oka/Merck) (VARIVAX) injection (lot number W002599 which has been verified to be valid, expiration date reported and validated as 24-JAN-2024). All of the vaccines were administered at a dose of 0.5 milliliters (ml) as prophylaxis (strengths, routes of administration, and anatomical sites of injection were not reported). The administered dose of measles, mumps, and rubella (Wistar RA 27-3) virus vaccine, live (rHA) (M-M-R II) and hepatitis A vaccine, inactivated (VAQTA) were stored at a temperature of 65 degrees Fahrenheit (F), during a time frame of 17 hours; the dose of varicella virus vaccine live (Oka/Merck) (VARIVAX) was stored at a temperature of 67 degrees F, during a time frame of 17 hours (product storage error). The patient's medical history included gastroesophageal reflux disease, candida infection and fracture of clavicle due to birth trauma. Her concurrent condition, and concomitant therapies were not reported. On 30-JUN-2022, the patient was vaccinated with a pediatric dose of hepatitis A Vaccine, inactivated (VAQTA), valid lot# U038841, expiration date 06-MAR-2023, on the left thigh (dosage regimen and route of administration were not provided) (Inappropriate age at vaccine administration) instead of pediatric dose of hepatitis B Vaccine (Recombinant) (RECOMBIVAX HB) (wrong vaccine administered).
