Client immediately responsive but sleepy, able to answer questions but sleepy. Oxygen saturation levels 99-100%. Client given sips of water and fruit snacks and after eating several fruit snacks, client returned to baseline.

Days after this vaccine was given, another record was received by the school nurse that showed patient was not in need of MMR or Varicella and was up to date on these vaccines as 2nd dose was given in 2011. Patient had an additional dose of MMR and Varicella on 6/22/22 and was given a combination vaccine that is not intended for age 16
