Symptomtext
Through an error in communication, I administered a flublok vaccine to a patient when he was expecting a pfizer vaccine. After receiving flublok, patient stated that he was allergic to eggs and couldn't get flu vaccine. After apologizing, I explained that flublok was an egg free vaccine. I then administered Pfizer vaccine and counseled patient on possible adverse effects from receiving both Pfizer and flublok. To my knowledge, patient never reported any adverse events to this pharmacy. He may have reported elsewhere. I reviewed vaccination protocol with my pharmacy manager and technician who was involved in the error.
