Symptomtext

Anaphylaxis about 15 minutes after the injections. Chest pain, shortness of breath, hives, pain with swallowing. She was given an EpiPen, put on oxygen, vital signs monitored and were stable. Patient was transported to a local emergency room. Recovered. Referred to allergist for further evaluation

Symptomtext

Patient received Twinrix and Typhoid injection for travel. Within 3-4 minutes the patient fainted and had a mild seizure with body stiffness, shaking and rapid eye movement.

Symptomtext

Approximately 45min after receiving the vaccines above he noticed the gradual onset of a pruritic erythematous rash affecting primarily his trunk with spread to his extremities as well as his face (perioribital, perioral). No mucosal involvement. Followed up with his primary care provider who provided topical therapies and systemic (benadryl) for dermatologic manifestations. Reports that within weeks he also began to develop dyspnea on exertion, asthma like symptoms, and resting tachycardia that have all resulted in permanent need for medical therapies to control his symptoms. Reports he is receiving ongoing evaluations with an Allergist, Cardiologist and Pulmonologist.

Dermatitis Dyspnoea Rash pruritic

Patient reported on 1/6/2023 tates that within 1 day of receiving vaccinations last year he noticed the onset of an itchy rash without weeping or vesicles on his arm; he states he has been afflicted with a chronic dermatitis since with a 1 year history of itching, migrating from neck, face, eyes, arms; he also reports occassional difficulty breathing since that date and attributes it to the vaccination and he uses an albuterol inhaler PRN; he states he went to the ED and was referred to subspecialty care but did not get a formal assessment from an allergist or pulmonologist because he ran out of benefits. For his itchy rash he is now using atarax PRN and zyrtec PRN.

Per notes from medical provider: ?HPI Patient comes in for evaluation of rash. Onset of rash 315. Had just received immunizations including COVID booster, yellow fever, typhoid and flu vaccine 215. Rash is itchy and red. Rash is generalized and spread to his chest back arms neck abdomen and legs. Rash is itchy. States his throat feels irritated and scratchy. States he did have nausea but that is better. Denies chest pain or airway closure. States he did take some ibuprofen. He denies body aches, headache, syncope. Denies history of allergic reactions to vaccines in the past. Mother accompanies patient here in the clinic. Allergic Reaction This is a new problem. The current episode started today. The problem occurs daily. The problem has been rapidly worsening since onset. Associated symptoms include coughing (dry) and a rash. Pertinent negatives include no abdominal pain, chest pain, eye itching, eye redness, stridor, trouble swallowing, vomiting or wheezing. There is no history of food allergies. Encounter Diagnoses -T80.69XA Allergic reaction to vaccine -L50.9 Hives -L29.9 Pruritus Patient was seen today for allergic reaction. Diagnoses and all orders for this visit: Allergic reaction to vaccine - methylPREDNISolone sodium succinate(PF) (SOLU-MEDROL) injection 40 mg - diphenhyrAMINE (Benadryl) capsule 25 mg - famotidine (PF) (Pepcid) injection 10 mg - EPINEPHrine (EPIPEN) 0.3 mg/0.3 ML Atln; Inject 0.3 mLs (0.3 mg total) into muscle 1 (one) time a day as needed (anaphylaxis). Or subcutaneous. May repeat does x 1 after 5-15 min Educated mother she can give another Benadryl 25 mg if needed. Educational handout given on medications. Educated on EpiPen. If airway closure did educate on administration of EpiPen and to go to the ER immediately or call 911. Mother in agreement with plan and verbalized understanding. Severe headache one hour post vaccine. Complaining of pain in forehead and stinging sensation. Piercing scream have never heard her cry like this in pain.

Patient was weak, sleepy, with temperature to 100, cold and with chills for the duration of the day. She took naps, lost appetite and was not feeling well starting in the early afternoon (approximately 4-6 hours after receiving the injection).

Symptomtext

Patient presented to the clinic for typhoid injection due to traveling to Africa in 3 weeks. Patient denies any adverse reactions to vaccines in the past or fainting or passing out after receiving a vaccine in the past. VIS provided. Patient left clinic without incident, Patient continued to go shopping for probiotics in the store about 20 min later her husband knocked on the clinic door stating my wife feels like she is going to pass out. Found patient laying awake , alert and oriented on the floor. Patient and husband denied any LOC patient stated she felt very weak and dizzy husband helped lower her to the floor and denied any falls or bumping her head. PAtient stated she felt very weak, dizzy , hot / cold also described a tight sensation in her nasal passages. Denied any SOB , CP , throat tightness no signs of swelling or rash noted. NP continued to st ay with patient and instructed husbands to get juice, water, and a granola bar (pt only had breakfast in the AM and a few snacks around 11 am did not have lunch yet, patient VS were 110/78 , 99% o2 sat, hr 75 and temp 97.8 RR 20 , after 20 min attempted to get patient to stand patient stood up but felt very dizzy a chair was taken from pharmacy and placed behind her was assisted with husband in the chair patient continued to have the above symptoms despite 45 min time elapsing discussed and agreed to call 911 for further evaluation, Ambulance staff and police arrived around 2:15 pm - blood sugar was evaluated was 135 patient and husband agreed to go to the Valley ER via ambukance for further assessment and evaluation transported around 2:40 pm
