Syncope Tinnitus

Symptomtext

After receiving all three vaccines (HPV, MCV4 and Tdap) patient fainted. Patient was awaken by RN that administered the vaccine by using an alcohol swab and when the patient was stable he was taken to the nurses office by RN. Vital signs were taken B/P: 116/74 and HR: 101. Patient was given to juices and the RN stated the patient did complain about ringing in the ears otherwise was feeling better. Patient was picked up by his grandmother and she explained to the RN that he is afraid of needles.

Feeling abnormal Pain Vomiting

Symptomtext

Minutes after receiving vaccines, the child vomited, stated she feels funny in her legs and c/o body aches. School personnel called her mother. The mother picked up child and stated she would take her to the ER.

General fatigue, nausea and lip swelling started about 1-2 hours after injections. Given Benadryl per advice of on -call physician. Symptoms improved but not resolved until following day

12 year old patient administered expired dose of MENACTRA vaccine with no reported AE; Initial information received on 07-Jul-2021 regarding an unsolicited valid non-serious case received from a other health professional via Physician. This case involves a 12 year old and unknown gender patient who was vaccinated with an expired dose of suspect MENINGOCOCCAL A-C-Y-W135 (D CONJ) VACCINE [MENACTRA]. Medical history, medical treatment, vaccination and family history were not provided. Concomitant medications included HPV VACCINE VLP RL1 4V (YEAST) (GARDASIL) for Immunisation; and DIPHTHERIA VACCINE TOXOID, PERTUSSIS VACCINE ACELLULAR, TETANUS VACCINE TOXOID (TDAP) for Immunisation. On 28-May-2021, the patient received a 0.5 ml dose of suspect MENINGOCOCCAL A-C-Y-W135 (D CONJ) VACCINE (lot U6575AB and expiry date: 08-May-2021) (Frequency: once) via an intramuscular route at an unknown administration site (expired product administered) for prophylactic vaccination. It was an actual medication error case due to expired vaccine used (latency: same day). It was also reported Caller stated that a dose of MENACTRA was given on 5/28/2021 but it had expired on 5/8/2021. If baby was delivered: delivery date and if baby experienced an adverse event. Inform that they will be receiving a follow up questionnaire. If calling specifically about being added to the pregnancy registry: all pregnancy cases are added to the registry, it is an FDA requirement. Product used: Used At the time of reporting, no adverse event was reported. This suspected adverse reaction report is submitted and classified as a medication error solely and exclusively to ensure the marketing authorization holder's compliance with the requirements set out in the Directive 2001/83/EC and Module VI of the Good Pharmacovigilance Practices. The classification as a medical error is in no way intended, nor should it be interpreted or construed as an allegation or claim made by the marketing authorization holder that any third party has contributed to or is to be held liable for the occurrence of this medication error.

a medical assistant gave an expired dose of Menactra; which expired 08May2021 with no adverse event; Initial information was received on 02-Jun-2021 regarding an unsolicited valid non-serious case from a other health professional. It was reported that a medical assistant gave an expired dose of MENINGOCOCCAL A-C-Y-W135 (D CONJ) VACCINE [MENACTRA] (Suspension For Injection) (lot number: U6575AB, expiry date: 08-May-2021) to a 12 year old female patient on an unknown date via an unknown route for prophylactic vaccination (expired product administered). It was a case of actual medication error due to expired vaccine used. Concomitant medication included sodium. At the time of report, no adverse event reported. This suspected adverse reaction report is submitted and classified as a medication error solely and exclusively to ensure the marketing authorization holder's compliance with the requirements set out in Directive 2001/83/EC and Module VI of the Good Pharmacovigilance Practices. The classification as a medical error is in no way intended, nor should it be interpreted or construed as an allegation or claim made by the marketing authorization holder that any third party has contributed to or is to be held liable for the occurrence of this medication error.
