Provider documentation of event provided below: Pt has an adverse reaction to a vaccine that she received at visit (likely HPV). She got up to go to the bathroom and felt dizzy. She because weak and unresponsive. She was taken to room by staff safely. Vitals taken. HR was low in the low 70;s initially. BPs normal. Patient was in and out of responsiveness. She never had any trouble breathing. Evaluated every 20 min. Before leaving was alert and oriented X3. Was eventually able to walk out of the clinic without dizziness. Tylenol was given for headache. ER precautions printed and sent home with father.

Symptomtext

Patient was given an expired dose of Menactra with no adverse event; Initial information regarding an unsolicited valid non-serious was received from a Consumer/non-hcp (healthcare professional) via Medical Information (Reference number- 00618521) and transmitted to Sanofi on 27-May-2021. This case involves a 17 years old female patient who received an 0.5 mL expired dose of suspect MENINGOCOCCAL A-C-Y-W135 (D CONJ) VACCINE [MENACTRA] (lot U6596AA and expiry date: 24-May-2021) via unknown route in unknown administration site (expired product administered) for prophylactic vaccination on 27-May-2021. The patient's medical history, past medical treatments, past vaccinations and family history were not provided. Concomitant medications included POLIO VACCINE (POLIO VACCINE) and HEPATITIS B VACCINE (HEPATITIS B VACCINE) for Immunisation. It was an actual medication error case due to expired vaccine used (latency: same day). At the time of reporting, the patient experienced no adverse event. This suspected adverse reaction report is submitted and classified as a medication error solely and exclusively to ensure the marketing authorization holder's compliance with the requirements set out in the Directive 2001/83/EC and Module VI of the Good Pharmacovigilance Practices. The classification as a medical error is in no way intended, nor should it be interpreted or construed as an allegation or claim made by the marketing authorization holder that any third party has contributed to or is to be held liable for the occurrence of this medication error.
