Symptomtext

Patient vaccinated and waiting on twin sibling to be vaccinated. After sibling was vaccinated patient stated she felt "funny" (probably less than 10 minutes after she received the vaccinations). Within a few seconds patient fell to the floor and passed out. Her mother (who is a nurse) held her and spoke to her while I got an ice pack for her. She came around, had some water and sat for a few minutes and said she was feeling better. Spoke with patient's mother later in the afternoon and she was feeling fine.

Dizziness Syncope

Symptomtext

After finishing his visit and receiving vaccinations, the individual accompanied by his mother at the reception told his mother that he felt a little dizzy, the mother expressed that it was probably because he had not eaten and suddenly the child began to get dizzy. The individual fainted without answering his mother's questions. Immediately without waiting, the receptionist and the staff called the doctors, and the child was transferred to the exam room. Vital signs were taken, alcohol was used to awaken from fainting, the individual had normal vital signs, and his reaction to fainting was momentary without consequences. The individual was evaluated and sent home in good health by Dr, MD (pediatrician).

Crying Fear Headache Palpitations Tearfulness

Symptomtext

1315-approx. 10 minutes after administration patient c/o headache, states she feels like her heart racing with palpitations. Pt is awake and alert, oriented, conversational and able to follow commands. Resp even and unlabored, rate 16. Skin warm and dry with slight pallor. Pt assisted from chair where she was seated to gurney and encouraged to lay down, and breath deeply; pt complied. Blood pressure obtained and recorded; results wnl for age and situation. Pt became tearful, stated she was crying due to h/a 5/10 and states she is scared. Patient reassured and cool moist compress applied to forehead. Continued monitoring in observation area for additional 15 minutes, during this time Patient able to sit up, at a banana, drink water, talk with parent and siblings, no longer tearful. At reassessment pt awake and alert, oriented, respiratations even/unlabored, 18/min; Skin warm and dry, normal color, no pallor noted; states she feels better, denies pain. Parent states she has episodes of anxiety, during which she has similar complaints. VS again obtained and WNL. Pt able to stand and walk, denies lightheaded, no s/s of syncope; no s/s discomfort or distress. No further complaints from pt at this time. Family instructed on post-vaccine comfort measures before departing. Medical Director notified

Day after receiving vaccine, patient developed site reaction - right upper arm very red and warm , swollen from the shoulder to mid bicep. Pain going to the elbow. Underneath the reddened area is a two inch square hard area. A low grade fever and general malaise. Patient self monitored and treated at home with ice, Tylenol.

Symptomtext

Pain that was extreme and miscular 9 out of 10 over my entire body. The arm where I had the shot was most painful but it. Was not red or swollen. No joint swelling. The pain decreased to 6 out of 10 with Tylenol. I also had joint pain in the feet, the knees, the elbows, the fingers and the shoulders. Also had a sinus Headache that resolved with my nasal spray. I felt like I had the flu. No fever just aches this is still going on but it's reducing and I just started steroids today.

I was recently diagnosed as having covid in February since that time my allergies have been so bad that I had to wear a mask when I went outside in the warmer months and I have been on increased allergy medications. I never had allergies that bad in my life. I had achiness with flu vaccine in 2019 and thought that maybe I had a touch of the flu.after.getting the vaccine, but the symptoms only lasted about 15 hours. I have asthma and was tapering off the medicines till some time in Jamiary 2020

Patient noted a blister like lesion in the area of the vaccination on the left upper arm. First noted a day after the vaccination. Did also have expected soreness and erythema to the area. Seen two days after the vaccination and a 0.5 cm flaccid blister noted lateral to the injection site but in the area of the part of the bandaid. Dad states blister is bigger today than it was last evening

provider in the office gave a 5 year old child a dose of ADACEL with no adverse event; Initial information regarding an unsolicited valid non-serious case was received from other health professional via Agency (Reference number- 00676275) and transmitted to Sanofi on 09-Jul-2021. This case involves a 5-year old male patient for whom it was reported that provider in the office gave dose of DIPHTHERIA-2/TETANUS/5 AC PERTUSSIS VACCINE [ADACEL] (product administered to patient of inappropriate age). It was unknown if the patient had any medical history, concomitant disease or risk factor. Past medical treatment(s), concomitant medication(s), vaccination(s) and family history were not provided. On 06-Jul-2021, the patient received 0.5 ml once dose of suspect DIPHTHERIA-2/TETANUS/5 AC PERTUSSIS VACCINE (lot number: U6817BA and expiration date: 18-Sep-2022) via intramuscular route in the right deltoid for prophylactic vaccination. It was a case of actual medication error due to inappropriate age at vaccine administration (latency: same day). It was reported that " Caller is requesting to know what she should do and if the child needs to have another vaccination to replace it. Patient Information o Patient age <1 year enter as 2m, 4m, etc. Many pediatric vaccines need month for age". At the time of report, no adverse event reported. This suspected adverse reaction report is submitted and classified as a medication error solely and exclusively to ensure the marketing authorization holder's compliance with the requirements set out in Directive 2001/83/EC and Module VI of the Good Pharmacovigilance Practices. The classification as a medical error is in no way intended, nor should it be interpreted or construed as an allegation or claim made by the marketing authorization holder that any third party has contributed to or is to be held liable for the occurrence of this medication error.

This case involves a 1 months old male patient who was given DIPHTHERIA-2/TETANUS/5 AC PERTUSSIS VACCINE [ADACEL] (lot number U6817BA ad expiration date 18-sep-2022) in error instead of the intended HEPATITIS B VACCINE (lot number and expiration date not reported),via unknown route in the right vastus lateralis for prophylactic vaccination on 02-Mar-2021 (wrong product administered) (product administered to patient of inappropriate age). The patient's past medical history, medical treatment(s), vaccination(s) and family history were not provided. No concomitant medication(s) were given to the patient. It was an actual medication error due to wrong vaccine administered and inappropriate age at vaccine administration (same day latency). It was also reported that registered nurse was calling about accidental administration of ADACEL. What information was available? How does Adacel compare to DTaP (diphtheria, pertussis, and tetanus)? also with a question that any information do the agency have about DTaP given to a child under the minimum age? It was not reported that the patient experienced any adverse event. This suspected adverse reaction report is submitted and classified as a medication error solely and exclusively to ensure the marketing authorization holder's compliance with the requirements set out in the Directive 2001/83/EC and Module VI of the Good Pharmacovigilance Practices.
