Symptomtext

Patient came with her mother to get the meningococcal vaccine. After the vaccine mother was waiting for immunization documentation while the patient left the immunization area and was seated in the stores health self-screening machine next to her mother. Patient fainted and she was immediately laid flat on her back at which point she recovered and stood up. Code white was called, but not 911. They where asked to remained in the pharmacy area for approx 20 minutes seated which they complied. When they were ready to leave the store manager brought over a wheelchair and she was wheeled to their vehicle.

Three to four minutes after the Patient received the above vaccines, he became pale and diaphoretic and showed seizure like symptoms.

expired dose with no adverse event; Initial information received on 18-Feb-2022 regarding an unsolicited valid non-serious case received from a other health professional. This case involves a 11 years old female patient who took expired dose with no adverse event while receiving vaccine Meningococcal A-C-Y-W135 (D Conj) Vaccine [Menactra]. The patient's past medical history, medical treatment(s), vaccination(s) and family history were not provided. First time product used: Yes Concomitant medications included Meningococcal Vaccine B (Meningococcal Vaccine B) ; Hpv Vaccine (Hpv Vaccine) for ; and Diphtheria Vaccine Toxoid, Pertussis Vaccine Acellular, Tetanus Vaccine Toxoid (TDAP) all for Immunisation. On 17-Feb-2022, the patient received dose of 0.5 ml once a day for suspect Menactra (Batch number U6828AA expiry 09-Jan-2022) via intramuscular route in the right deltoid for immunisation. On 17-Feb-2022, it was reported that patient was administered an expired dose with no adverse event (expired product administered) of Menactra. Action taken was not applicable Outcome was Unknown This suspected adverse reaction report is submitted and classified as a medication error solely and exclusively to ensure the marketing authorization holder's compliance with the requirements set out in the Directive 2001/83/EC and Module VI of the Good Pharmacovigilance Practices. The classification as a medical error is in no way intended, nor should it be interpreted or construed as an allegation or claim made by the marketing authorization holder that any third party has contributed to or is to be held liable for the occurrence of this medication error.

inadvertently received a Menactra in her pharmacy on 11-Jan-2022 that had expired on 09-Jan-2022 with no reported adverse event; Initial information received on 11-Jan-2022 regarding an unsolicited valid non-serious case received from a pharmacist. This case involves 43 years old female patient who inadvertently received a Menactra in her pharmacy on 11-Jan-2022 that had expired on 09-jan-2022 with no reported adverse event while receiving vaccine MENINGOCOCCAL A-C-Y-W135 (D CONJ) VACCINE [MENACTRA]. The patient's past medical history, medical treatment(s), and family history were not provided. Concomitant medications included DIPHTHERIA VACCINE TOXOID, PERTUSSIS VACCINE ACELLULAR, TETANUS VACCINE TOXOID (TDAP) for Prophylactic vaccination. On 11-Jan-2022, the patient received a 0.5mL dose of suspect MENINGOCOCCAL A-C-Y-W135 (D CONJ) VACCINE (lot U6828AA; Strength: standard; frequency: once; expiry date: 09-Jan-2022) via intramuscular route in the left arm for prophylactic vaccination. On 11-Jan-2022 the patient developed a non-serious event "inadvertently received a menactra in her pharmacy on 11-Jan-2022 that had expired on 09-Jan-2022 with no reported adverse event" (expired product administered) (latency: same day). Action taken with MENINGOCOCCAL VACCINE (MENACTRA) was not applicable. At time of reporting, the outcome was Unknown for the event inadvertently received a menactra in her pharmacy on 11-jan-2022 that had expired on 09-jan-2022 with no reported adverse event. This suspected adverse reaction report is submitted and classified as a medication error solely and exclusively to ensure the marketing authorization holder's compliance with the requirements set out in the Directive 2001/83/EC and Module VI of the Good Pharmacovigilance Practices. Symptomtext

When looking at the MCIR I pulled out the Menactra instead of the MMR that should have been given to the patient at the time of the apt. When ordering the injection in the chart I did order MMR and pulled the wrong vaccine and administered this to the patient. I did call the health department right away to find out if this was okay for the patient besides it not counting toward his immunizations since it was done early. I also contacted the patient and spoke with the mother and let my office manager know of the events that occurred .
