Patien received menectra and boostrix vaccine at the same time. Within 5 minutes, she manifested seizure for a very short time. Patient's mom who was with her, called pharamcy staff for help. When we saw patient, she was conscious and laying down on the floow. Nurse practitioner came and checked her vitals. Temperature 98.0F Oxygen level 99%. Pulse:54 BP 98/52 We called 911. 911 team came and checked on her. A while later, the patient walked home.

Intern admnistered boostrix in one arm and menactra in the other. He stated patient was a little nervous and after giving the second vaccine the patient started to feint. Intern layed patient on the groud and called me. When I arrived patient was concious but was hyperventilating and still feeling dizzy. Raised the patient's feet and tried to get her to breath slower and called mergency services. With the help of the nurse next door we got her BP (98/56), pulse (98), temperature (99.4). Patient also started to feel cold so we got her a blanket. Eventually patient was able to sit on the chair with a bit of help. When emergency services arrived at the scene they took vitals again and patient's caregivers declined a ride to the hospital. Patient and family left shortly after emergncy services.

Patient received all vaccines without difficulty. Approximately 2 minutes after the last vaccine given, patient experienced a vasovagal reaction associated with an episodic tremor (lasted approximately 5 seconds). Patient did not lose consciousness. Patient was placed in a comfortable position, given juice, and observed for 30 minutes. Symptoms improved and patient was picked up from the the health center by parent (mother).

This spontaneous report was received from a nurse prescriber referring to a 11-year-old female patient. Concomitant therapies included MENACTRA and ADACEL. The patient had no pertinent medical history or drug reactions/ allergies. On 29-MAR-2021 the patient was vaccinated with the first dose of GARDASIL 9 (lot # T026954, expiration date 13-APR-2022, 0.5 milliliter, once) via intramuscular for vaccination. On 29-MAR-2021, the patient experienced seizure like tonic-clonic movement with syncope after receiving first dose of GARDASIL 9 along with MENACTRA and ADACEL. The patient recovered after 15 minutes (on 29-MAR-2021). The patient was given cool compress and maintained a supine position as treatment. No lab diagnostics/studies were performed. The patient had sought medical attention. The events were not cancer, and the patient was not overdose. Device was not available for evaluation (obtain was false). The causality between GARDASIL 9 and the events was not reported. Upon internal review, seizure like tonic-clonic movement with syncope were determined to be medically significant. After administration of vaccines- pt c/o itchy skin, was scratching her bilateral forearms with redness noted with few red papules, no urticaria. Given 50 mg Benadryl ( 20 ml of liquid Benadryl 12.5 mg/5 ml) with no improvement and pt stating her throat felt itchy, Given Epipen 0.3 mg in left thigh at 17:37. No change in pt symptoms, vital signs stable. Pt ongoing c/o itchy throat, SPB from 140's to 120's , O2 sat from100 to 95% so second dose of Epinephrine 0.3 mg given IM in right thigh at 17:49 pm. Ambulance crew arrived and took over care of pt- transported to Hospital.

The patient's past medical history, medical treatment(s), vaccination(s) and family history were not provided. Concomitant medications included DIPHTHERIA VACCINE TOXOID, PERTUSSIS VACCINE ACELLULAR, TETANUS VACCINE TOXOID (TDAP) for Prophylactic vaccination; HPV VACCINE (HPV VACCINE) for Prophylactic vaccination; and INFLUENZA VACCINE (INFLUENZA VACCINE) for Prophylactic vaccination. On 01-Mar-2022, the patient received a dose of suspect MENINGOCOCCAL A-C-Y-W135 (D CONJ) VACCINE lot U6874AA and expiry date 21-Feb-2022 via intramuscular route in unknown administration site for prophylactic vaccination. On 01-MAR-2022 the patient developed a non-serious event "expired vaccine administered, with no reported adverse event (expired product administered) (latency same day) following the administration of MENINGOCOCCAL A-C-Y-W135 (D CONJ) VACCINE. Action taken with MENINGOCOCCAL VACCINE (MENACTRA) was not applicable. It was not reported if the patient received a corrective treatment for the event (expired vaccine administered, with no reported adverse event). This case involves an 18 years old male patient who got mistakenly vaccinated with menactra and later on they realized that the lot number expired on 21feb2022 with no reported adverse event while receiving vaccine Meningococcal A-C-Y-W135 (D Conj) Vaccine [Menactra]. The patient's past medical history, medical treatment(s), vaccination(s) and family history were not provided. Concomitant medications included Meningococcal Vaccine B (Menb) for Prophylactic vaccination; and HPV Vaccine (HPV Vaccine) for Prophylactic vaccination. On 18-Mar-2022, the patient received 0.5 mL dose of Meningococcal A-C-Y-W135 (D Conj) Vaccine (form, strength, expiry date: Unknown; lot U6874AA) via intramuscular route in the right deltoid for Immunization. On 18-MAR-2022 (Latency: same day), there was a mistake of vaccinating the patient with menactra and later on they realized that the lot number expired on 21feb2022 with no reported adverse event (expired product administered). patient presented to the office for second dose of HPV vaccine. Patient was administered Meningococcal Conjugate MCV4P by mistake. Patient was not due for second dose of vaccine given until age 16. Patients parents were notified immediately. Patient did not and has not experienced any side effects
