Dizziness Fall Loss of consciousness

Symptomtext

Pt was given 3 vaccines, pt was ambulatory and alert after shots were administered. However about 5 minutes after, pt stated that he felt little light headed and shortly after he collapsed. Pt was caught by one of our nurses before he hits the ground. Pt lost consciousness without any respiratory/ cardiac distress for 30 seconds. Shortly after, pt became alert and was moved to an exam bed for vital signs and observation. Vital signs were all normal, no fever, no respiratory/cardiac distress, and pt was fully alert. Pt was examined for another 30minutes and was discharged after doctor cleared the pt.

Symptomtext

4/1/2021 1135 - Administered Menactra and Tdap in client's right deltoid in that order. About 10 seconds after receiving Tdap, client's eyes rolled back in head, skin paled, and client lost consciousness. RN and mother supported client upright in chair. Client had 5 involuntary muscle spasms of upper torso. 30 seconds after losing consciousness, client regained consciousness without intervention. Nurse and mother assisted client into recovery position on the floor. Another nurse retrieved cold washrag, cold water, and graham crackers. Cold washrag applied to back of the neck. After 2 minutes on the floor, client's color returned and client stated feeling better. Nurse administered HPV into left deltoid with client in side-lying position on floor, no reaction. Mother stated client had never had a syncopal episode with vaccinations before, but has another child prone to syncopal episodes. When asked if client had breakfast, both client and mother said no. About 5 minutes after receiving HPV vaccine, client felt well enough to move into a sitting position, tolerated graham crackers and water. After 5 minutes in sitting position, client felt able to stand. Client stood for 2 minutes and stated feeling fine. Client escorted by mother out of clinic.

Symptomtext

After receiving the flu and menactra in L arm, then the HPV on the right arm, pt. Reported had witnessed jittery episode, turned pale, and had a near syncopal episode. MD immediately made aware. Pt. Alert , but pale. Blood pressures obtained 84/37 -> 94/46 HR 37 => after donut/juice 128/78 HR 67 => 125/62 (pt. Sitting up). POC glucose obtained, which was 106. - Pt reported not eating any food prior to this visit. - Improved mental status and skin color after given sugar.

Symptomtext

Mom called on Friday 8/6/21. She said her sons right arm is red, swollen and hot to touch from the vaccines he received on 7/22/21. He is complaining of his arm hurting him. Told mom this is not usual reaction 15 days after receiving the vaccines. Mom said child does not have fever. Mom said area on arm is large but does not extend from shoulder to elbow and is not swollen completely around arm. Instructed mom to take child to see his doctor and give us a call on Monday to let us know what doctor says. Mom did not call on Monday. Unable to get a response from mother.
