Symptomtext

Dad reports sycope with vaceine administration. lesions around ankles. Developed 1 small hive on L ankle over the next week more lesions appeared around ankles. Lesions began to widen and dusky purplish center appeared. Monitor and return if sx worsen.

Symptomtext

Pt. had a syncopal episode after vaccines were administered. She fell to the floor on her face from the exam table. She was unconscious fro a few seconds, lower lip had a slight abrasion, no tooth pain, no loose teeth (confirmed by her dentist), pain to nose and forehead, not bruising evident, slight amount of blood in both nostrils, no swelling. Patient stayed in the office until 9:40am. 3/29 Mom called requesting a nose xray. 3/31 a follow up call made to Mom. She had decided the xray would most likely not be necessary as pt. was doing better.

Symptomtext

About 4 hours after patient received vaccine, she started feeling dizzy and short of breath. Mom states that she looked pale and the areas around her lips were cyanotic. She also developed a rash on her chest and face. Mom gave her benedryl twice. She also developed a subjective fever on the same day. Mom gave some Tylenol and it resolved. Has not come back.

Symptomtext

Patient received both vaccines on 07.06.2021. Approximately 2 hours after she started having vomiting and abdominal pain. She preceded to the ER and was found to have gallbladder inflammation with evidence of chronic cholecystitis. Patient had a laparoscopic cholecystectomy on 07.07.2021.

Symptomtext

Macular blanching rash. It appeared less than 24 hours after the vaccination. She did not treat the rash. The rash hurt and itched. She has felt like she was freezing cold all morning. Prescribed triamcinolone cream TID and tylenol. The rash resolved.

patient receiving an expired dose of menactra, with no AE (adverse event); Initial information received on 05-May-2022 regarding an unsolicited valid non-serious case received from a other health professional. This case involves a 11 years old male patient who received an expired dose of menactra, with no ae (adverse event) while receiving vaccine meningococcal a-c-y-w135 (d conj) vaccine [Menactra]. The patient's past medical history, medical treatment(s), vaccination(s) and family history were not provided. Concomitant medications included HPV vaccine. On 03-May-2022, the patient received a 0.5ml dose of suspect meningococcal a-c-y-w135 (d conj) vaccine unknown formulation and strength lot U6919AB via intramuscular route in the left deltoid for prophylactic vaccination. Same day, the patient received an expired dose of menactra, with no ae (expired product administered). After administering the vaccine I was documenting in the pts chart I realized at that time the vaccine had expired. I immediately reported to the Dr. Pt was notified . He had no adverse reactions . He followed up with the provider today . He still had no adverse reaction . Pt was given second dose of Meningococcal Conjugate . He was instructed if he had any adverse effects to report to us immediately.
