Symptomtext
Patient was not given the complete dose of Pentacel which caused the dose to be invalid. The patient was only given the Pentacel Solution of the vaccine. Therefore per the Provider to make the dose valid patient was given then a dose of HIB LT Lot:UJ584AA to valid the correct dose. Patient also received Prevnar RT Lot:EN8811 which the patient was due for at the time of the visit.
