Symptomtext

Pt. had received his vaccines, and at check out, pt. fainted. He hit his right parietal/occipital area on the linoleum floor from a standing position. The nurses reported some jerky motions briefly after the head injury. He was assessed immediately afterwards by myself and looked pale in color and was conscious. There was no signs of external injury of his scalp. Patient was allow to lay in the recumbent position for a few minutes. He was sat up and given water and Sprite to drink. I reassessed him three times afterwards. The second time he had complained of red spots in his vision to the nurses which had resolved on my assessment. He had some blurriness of vision. The third time he complained of a HA and was given ibuprofen 300 mg. The blurriness of vision had resolved. Prior to discharge, pt. was conversant. His was discharged home with his mother to observe him for the next 24 hours. Mother reported several episodes of vomiting after leaving office. Advised to proceed to the local Emergency Room. Arrived there at 1232pm for evaluation.

Symptomtext

Pt has a history of feeling weak and passing out with previous dose of HPV9 vaccine and with getting blood drawn. Pt lied down on exam table prior to administration of vaccinations. A few minutes after administration of vaccines, pt c/o feeling weak, tired, hot and nauseated, pt became pale and diaphoretic. Vital signs monitored for 30 minutes, pt given water, candy and an ice pack applied to forehead and neck and then pt reported feeling better with vital signs returning to normal and color returned to face and lips within 30 minutes.

This case involves an unknown age and gender patient while received MENINGOCOCCAL A-C-Y-W135 (D CONJ) VACCINE [MENACTRA] then reported patient did not received a full dose due to the patient squirming during injection (underdose) and the vaccine did visibly leak down the patient's arm (exposure via skin contact). Medical history, medical treatment, vaccination, concomitant medication and family history were not provided. On 17-Aug-2021, the patient received a 0.5 mL (total) dose of suspect MENINGOCOCCAL A-C-Y-W135 (D CONJ) VACCINE (solution for injection) [lot U7140AA, expiry date 04-Nov-2022] via unknown route at an unknown administration site for prophylactic vaccination and reported (latency was on same day) patient did not received a full dose due to the patient squirming during injection (underdose) and the vaccine did visibly leak down the patient's arm (exposure via skin contact). Details of laboratory data not reported. It is unknown if the patient experienced any additional symptoms/events. No adverse event was reported. It was not reported if the patient received any corrective treatment. The outcome was unknown for both the events.

This case involves a 17 years old male patient who received an expired dose of MENINGOCOCCAL A-C-Y-W135 (D CONJ) VACCINE [MENACTRA] with no reported adverse event. The patient's past medical history, vaccination(s) and family history were not provided.Patient had no medical treatment and no concomitant medication. On 07-Nov-2022, the patient received a 0.5ml, 1 X(once) dose of suspect MENINGOCOCCAL A-C-Y-W135 (D CONJ) VACCINE (lot U7140AA and expiry date:04-Nov-2022) via intramuscular route in unknown administration site for Immunization. On 07-Nov-2022 the patient developed a non-serious event of patient received an expired dose of menactra with no reported adverse event (expired product administered) (Latency: same day) following the administration of MENINGOCOCCAL A-C-Y-W135 (D CONJ) VACCINE. Action taken : not applicable. This case involves a 17 years old male patient who experienced extra dose of menactra was administered with no reported adverse event while receiving vaccine MENINGOCOCCAL A-C-Y-W135 (D CONJ) VACCINE [MENACTRA]. The patient's past medical history, medical treatment(s), vaccination(s) and family history were not provided. Concomitant medications included MENINGOCOCCAL VACCINE B RFHBP/NADA/NHBA OMV (BEXSERO); and MENINGOCOCCAL VACCINE A/C/Y/W CONJ (CRM197) (MENVEO). On 18-Feb-2022, the patient received a dose of suspect MENINGOCOCCAL A-C-Y-W135 (D CONJ) VACCINE lot U7140AA (expiry date 04-Nov-2022) via intramuscular route in the right deltoid for Immunization. On 18-MAR-2022 the patient developed a non-serious event "extra dose of menactra was administered with no reported adverse event "(extra dose administered) 1 month following the administration of MENINGOCOCCAL A-C-Y-W135 (D CONJ) VACCINE. Action taken with MENINGOCOCCAL VACCINE (MENACTRA) was not applicable.
