Clinic nurse was giving routine vaccines. She gave HPV and flu in the right arm and was going to give Menactra in the left arm, when she gave it the patient passed out. She states that she missed lunch and hasnt ate anything since breakfast. The patient was given some cracker and a drink and regained full response back.

Patient received Tdap and Menactra vaccine and approximately 5 minutes later was dizzy, nauseated, diaphoretic and vomited times one. He was slow to respond to stimuli with increased pallor. VS taken, O2 applied and labs drawn. Blood glucose 110, continued to be weak, dizzy, and slow to respond. Sent to hospital via ambulance for evaluation. B/P 130/90, pulse 88. Monitored in ED for 1.5 hours then discharged with DX. vasovagal syncope.

Vaccine administered. Patient got up and ambulated to the check out desk. While there, he began to feel faint. He went to the bathroom and vomited. The nurses assisted him to ambulate to the nearest exam room and lay down. He stated that he blacked out. The nurses gave juice, crackers, and a cool washcloth. His blood pressure after that was still 88/54 in a sitting position. APRN was asked to see the patient at that point.

Patient was behind on vaccines and has never had some of the vaccines, according to her shot record. This was her first dose of Hep A, Hep B, MCV4, and MMRV (she has previously had measles and rubella, but not varicella vaccine or mumps). So mother brought patient in to catch up. Vaccines were given and approximately 5 minutes after as patient was walking out, she appeared to have a seizure and wet herself. EMS was called, it was strongly suggested that mom let them take her to the ER or take her herself, but in the end mom left with her daughter and it does not appear that child was taken to the emergency room as of yet. The doctor on-call at the clinic contacted mom to encourage her to take her to the ER, but it appears that mother did not take her.

nausea; vomiting; Patient received an extra of MENACTRA administered; Initial information was regarding an unsolicited valid non serious case was received from a consumer/non-health care professional via Information (Reference number- 00802399) and transmitted to Sanofi on 07-Oct-2021. This case involves a 14-year-old female patient who experienced nausea and vomiting after receiving MENINGOCOCCAL A-C-Y-W135 (D CONJ) VACCINE [MENACTRA] (extra dose administered). The patient's past vaccination(s) included MENACTRA on 18-SEP-2020. The patient's medical history, past medical treatment(s) and family history were not provided. Concomitant medications included ALTHAEA OFFICINALIS ROOT, CINNAMOMUM CAMPHORA ESSENTIAL OIL, CITRUS AURANTIUM OIL, EUCALYPTUS GLOBULUS LEAF ESSENTIAL OIL, MENTHA X PIPERITA ESSENTIAL OIL (GARDISIL) and DIPHTHERIA VACCINE TOXOID, PERTUSSIS VACCINE ACELLULAR 3-COMPONENT, TETANUS VACCINE TOXOID (BOOSTRIX). On 06-Oct-2021, the patient received a second dose of 0.5 mL suspect MENINGOCOCCAL A-C-Y-W135 (D CONJ) VACCINE (Solution for injection, lot U7140BA, expiry date 07-Oct-2022) via intramuscular route in the left arm for prophylactic vaccination. On 07-Oct-2021, the patient developed non-serious nausea and vomiting, 1 day following the administration of MENINGOCOCCAL A-C-Y-W135 (D CONJ) VACCINE. It was an actual medication error due extra dose administered (latency : the same day). At time of reporting, the outcome was unknown for the event. It was reported "their second dose of MENACTRA in the office on 06-Oct-2021 after receiving their first dose on 18-Sep-2020.This patient received the second dose 2 years too early. Patient has had nausea and vomiting which started on 07-Oct-2021". This suspected adverse reaction report is submitted and classified as a medication error solely and exclusively to ensure the marketing authorization holder's compliance with the requirements set out in the Directive 2001/83/EC and Module VI of the Good Pharmacovigilance Practices. Patient was present with her mother. At appox 3:15 pm mother came up to me with daughter present and showed me her left (deltoid) arm. Noted red rash around injection site (1/2 dollar size) warm to touch. No respiratory distress noted. Administered 2 tablets of Benadryl (25 mg each) per emergency policy and procedure @ 3:22 pm. With enough water for the patient to swallow the tablets. Their was no discomfort with swallowing noted. Vital signs taken every 5 min. 1) 3:22pm BP 98/74 P 80 R 16, T 97.9, Pulse Ox 98 % 2) 3:27pm BP 88/74 P 74 R 16, T 97.6, Pulse Ox 98%. 3) 3:32pm BP 102/64, P 79, R 16, T 98.2 Pulse ox 99%. Patient was laughing and talking with sibling the whole time. Mother was instructed on if the rash gets worse, Respiratory distress, Fever, the redness or tenderness where you got the shot gets worse notify her Doctor. I asked Mom if she would like the ambulance to come and check her out. She stated "No, I feel she will be okay". Notified Dr @ 3:50pm and Manager @ 6:50 pm. Patient left with mother and two siblings @ 3:55 pm no discomfort noted.
