Symptomtext

Patient vaccinated and waiting on twin sibling to be vaccinated. After sibling was vaccinated patient stated she felt "funny" (probably less than 10 minutes after she received the vaccinations). Within a few seconds patient fell to the floor and passed out. Her mother (who is a nurse) held her and spoke to her while I got an ice pack for her. She came around, had some water and sat for a few minutes and said she was feeling better. Spoke with patient's mother later in the afternoon and she was feeling fine.

expired pentacel administered with no reported adverse event; Initial information regarding an unsolicited valid non-serious case received via consumer/non-hcp on 16-Mar-2023. GMI number: 01535664. This case involves 4 months old female patient to whom expired Diphtheria/Tetanus/5 Hybrid AC Pertussis/IPV(VERO)/HIB(PRP/T) Vaccine [Pentacel (Vero)] administered with no reported adverse event. The patient's past medical history, medical treatment(s), vaccination(s) and family history were not provided. Concomitant medications included Pneumococcal Vaccine CONJ 13V (CRM197) (Prevnar 13) for Prophylactic vaccination; and Rotavirus Vaccine Live Reassort Oral 5V (Rotateq) for Prophylactic vaccination. On 16-Mar-2023, the patient was administered expired second dose suspect Pentacel vaccine Suspension for injection at 0.5ml dose once (lot: U719AA, expiry date: 28-Feb-2023) via intramuscular route in the left thigh for prophylactic vaccination with no reported adverse event (expired product administered) at same day latency. Action taken: Not applicable. No corrective treatment was received for the event. At time of reporting, the outcome was Unknown for the event expired product administered. This suspected adverse reaction report is submitted and classified as a medication error solely and exclusively to ensure the marketing authorization holder's compliance with the requirements set out in the guidelines. No adverse event/ No known pt adverse events; Indavertently administered improperly stored GARDASIL 9/ Temp excursion happened - Vaccines given despite; This spontaneous report was received from a licensed practical nurse (LPN) regarding to a 12-year-old male patient. The patient's concurrent conditions, pertinent medical history, concomitant therapies, drug reactions, and allergies were not provided. On 04-NOV-2021, the patient was inadvertently vaccinated with improperly stored doses of HPV rL1 6 11 16 18 31 33 45 52 58 VLP vaccine (yeast) (GARDASIL 9) second dose, intramuscularly in left deltoid, lot number S023475, with an expiration date of 26-MAR-2022 (dose, strength and formulation were not reported) for prophylaxis and meningococcal vaccine a/c/y/w conj (diptox) (MENACTRA) first dose, intramuscularly in right deltoid lot number: U7191AA, expiry date: 18-JAN-2023 (strength, dose and formulation, were not reported) for prophylaxis (product storage error). There were no additional adverse event details and no known patient's adverse events (no adverse event). Administered dose of HPV rL1 6 11 16 18 31 33 45 52 58 VLP vaccine (yeast) (GARDASIL 9) experienced the temperature excursion of 27.2 degrees Fahrenheit (F) and 49.8 F for 8 hours 30 minutes and 21 hours and 15 minutes respectively. There were no previous excursions reported. Temperature excursion details for meningococcal vaccine a/c/y/w conj (diptox) (MENACTA) were not provided. Digital Data Logger was involved.
