Narrative: Patient received Tdap and PPSV23 on 2/16/22 and developed rash on right arm where PPSV23 was administered the next day then on second day began to be SOB and was advised to go to ED outside the facility. Patient was treated for pericarditis and cellulitis. CRP and Sed rate were elevated but heart cath normal. Patient was treated with colchicine with good benefit and for cellulitis was treated with Keflex. Pt was hospitalized from 2/19 to 2/21 and on d/c was doing well, recovered, and no complications.

The patient received his vaccination 2-26-22, 2 days later on 2-28-22, when he returned to work he noticed that his left arm (the arm that received the vaccination) felt like it had pins and needles and numbness in his shoulder area. He also can not hold his arm up for more than a minute at a time before he has to put it down due to the pain. He has limited range of motion in his left shoulder now. He has been experiencing these symptoms every day since 2-28-22. He tries to take OTC pain relievers, such as Tylenol, to help manage the pain but he says that nothing helps. He haven't seen a provider yet, but has an appointment today (11-22-22) to see a provider for the condition.

The next day after injection developed a large knot on left upper arm. Area is very red and itchy. Sore to touch. Took Benadryl 25mg x 1 due to intense itching. There continues to be an area there almost a week later. Severe migraine 48 hours after injection and headache on and off for the next few days following.

ER: neg CXR, WBC 20K w/ bandemia, nl procal/lactate, micro hematuria, hypoNa, ALT 75. Felt better after IVF o 3/14 - Recurrence of sxs, seen in ER. Patient is G4P1(1,0,0,1) who was 32 weeks and 2 days gestation at the time of vaccination. No adverse effects to fetus have been noted. Patient received Adacel vaccination in right deltoid. Patient tolerated injection well. No pain at injection site. Since injection, patient has had worsening soreness around her shoulder/rotator cuff. Patient experiences great discomfort with any range of motion, such as putting on clothes or opening doors. Patient has been referred to physical therapy for this issue. If issue persists after the birth of her child, patient may undergo imaging of her shoulder but none has been completed at this point.

I feel my arm sore at the injection site start the night after receiving the vaccine. The next morning around 11am (~20 hours after injection), I start to develop symptoms including chills, low-temp fever (99~99.8F), sore joints around the hip and the spine, increase in heart rate (~70~80bpm at rest) during the day. . I took 2 doses of NyQuil (Acetaminophen) that evening but the symptoms persist and start to get worse, especially with the soreness at the injection site. The symptoms last until 48 hours after injection and were relieved after taking Advil (200mg, Ibuprofen). Despite most of my symptoms are relieved, the soreness at the injected arm lingers. I still have to take Advil occasionally.

patient received ADACEL last year and was mistakenly given another dose of vaccine with no reported adverse event; Initial information received on 27-Jul-2022 regarding an unsolicited valid non-serious case received from an other health professional. This case involves a 12 years old female patient who received Diphtheria-2/Tetanus/5 AC Pertussis Vaccine [Adacel] last year and was mistakenly given another dose of vaccine with no reported adverse event. The patient's past medical history, medical treatment(s), vaccination(s) and family history were not provided. On 14-Jul-2022, the patient received a 2nd dose 0.5 ML (once) of suspect Diphtheria-2/Tetanus/5 AC Pertussis Vaccine (lot U7266AA, expiry date;11-Jan-2024, strength; standard, formulation: unknown) via intramuscular route at Left deltoid for immunization. It was reported, the patient received adacel last year and was mistakenly given another dose of vaccine with no reported adverse event (extra dose administered) (onset 14-Jul-2022, latency: same day). Action taken: Not applicable. Outcome: Unknown. This suspected adverse reaction report is submitted and classified as a medication error solely and exclusively to ensure the marketing authorization holder's compliance with the requirements set out in the guidelines. The classification as a medical error is in no way intended, nor should it be interpreted or construed as an allegation or claim made by the marketing authorization holder that any third party has contributed to or is to be held liable for the occurrence of this medication error.
