Post-vaccination observation: immediately post-vaccination patient experienced vasovagal syncopal episode (10:42am) - patient placed in Trendelenberg position and provided ice packs. BP 88/52 HR 53 (10:45am). Instructed to wait additional 15 minutes for nursing observation - VS returned to normal BP: 101/68 HR 62 while sitting upright (11:02am). Provided patient water. After 30 min observation, patient stating he feels "good" and ok to leave the clinic. Patient left clinic in good health with his father accompanying him.

Pt arrived to the country approximately 7 days ago. As per mom patient has no immunization record. Adm. Hep B, IPV, Var, and Tdap to left arm, with no adverse reaction. Administered MCV4 to right deltoid, pt had an episode to syncope followed by a seizure that lasted one minute. Pt awake, alert and oriented X3. States she felt nausea and vomited. Pt was pale and diaphoretic. 11:30, laid pt down and elevated lower extremities. Pt's face and lip color returned to her normal. States she feels fine and requested to sit up. Mother requested for the last two vaccines to be administered. Mom states pt always faints with vaccines and the sight of blood. Administered MMR and HPV to right arm with no adverse reaction. Pt states she felt fine. Instructed mom to go to the ER in case of emergency. Mother verbalized understanding.

Following injections, child's arm became clonic and eyes rolled back, put child on the floor with legs elevated, diaphoretic. Child jumped up and stated does this, passes out. Sat child down on the bench and he became clonic again with arms and stared ahead. Eased child back down to the floor and the back of the childs head hit the floor. Resulted in a small elevated area on the posterior head. Vitals at 9:24AM were blood pressure left arm 120/40,Pulse 80, Respirations, child warm to touch, diaphoretic, face pale. Elevated legs, ice pack on the back of the head and cold compress on forehead. 9:35AM Vitals blood pressure 102/50, Pulse 78, Resp. 24, c/o being hot then cold. sat up the child and become lightheaded so layed back down at 9:40AM, Lips pale. 9:50AM blood pressure 94/30 pulse 72, Respirations 10 States not lightheaded, skin dry to touch and not hot. States feels better so was sitting and then would lay down in front of a fan. 10:01AM, blood pressure 120/72, Pulse 80, Resp. 12, states feeling better, lips red, skin warm to touch sitting up. Tried standing up and stated weak and sat back down on the bench. 10:04 AM blood pressure 120/70, Pulse 82, Resp 12, While sitting observed involuntary shaking of legs and arms. Child stated feeling better. at 10:15AM walked child to vehicle. Recommended to the adult with child that the child should rest the rest of this day.

Pt was given menquadfi first , then was given the hpv to the right arm. I turn and placed my syringe on counter and looked back to pt when he said I dont feel good, I stood right in front of him , asked him to take some deep breaths, which he did then he fainted into my arms. His arms were became rigid. Within 30 secs, he opened his eyes and said "what happened?" I told him that he fainted and he asked for his grandmother. he still didn't feel well. His skin was pale and diaphortic, he drank some water, I had him lye down on exam table for 15 - 20 min. he felt better after that . He did co,mplain of a Headache. He was able to ambulate to the car with his grandmother.

Patient presented to office for routine vaccination. Paperwork was completed and neither patient, nor father of patient indicated that patient had any previous reactions with vaccines or that patient was unwell when coming to clinic. Patient was to receive his next dose for Tdap and Menningitis vaccination. Patient reported that he was very nervous, but neither him nor his father had any questions for nurse. Patient tolerated vaccine administration well. Approximately 1 minute after the second vaccine was administered, patient stood up from his seat, remained standing for a few seconds, and then proceeded to fall backward against the wall. Patient was assisted to a lying position on the ground. Nurse attempted to rouse the patient, however patient was not responding verbally to nurse. Nurse noted no sign of trauma to patient's head. Nurse obtained vital signs on patient at this time to be: HR = 57 bpm, BP = 84/56 mmHg, and SpO2 = 97% RA. Patient's skin was slightly pale and clammy to the touch. During nurse's assessment of vital signs, patient became verbally responsive and began apologizing for the "fuss." Patient states, "It's happened before, where I've felt like this, but it's been a long time. I just got really dizzy like I do when I stand up too fast and then I'm so tall. How did I end up on the floor?" Nurse notified patient that he fell to the ground not long after standing up. Patient denied having any head pain, stating, "I just still feel kind of dizzy, but that's getting better, too." Patient was encouraged to continue to lie still. Patient's pupils were observed at this time using a pen light and were found to be round, equal, and reactive bilaterally. Patient reports, "I'm feeling a lot better, sorry about the fuss," and proceeded to sit up. Nurse advised patient to remain sitting and lean his back against the wall. Patient was given water at this time by other staff and encouraged to drink, which he did and finished the entire glass of water quickly. After additional questions from nurse, patient reports that he was very anxious before receiving his vaccines, did not have any breakfast, the only fluid he drank was one glass of milk, and he will occasionally get dizzy when he stands up too fast. Patient assured nurse that he was fine now, however nurse accompanied patient to a chair and encouraged him to sit for a few minutes so that she may recheck his vital signs. Patient and father of patient verbalized agreement. Another staff member gave more water to patient and encouraged him to drink. Vital signs were rechecked 5 minutes later and were found to be: HR = 74 bpm, BP = 110/76 mmHg, and SpO2 = 98% RA. Patient reported that he was feeling fine and apologized again. Nurse offered reassurance, but advised both patient and father of patient of ER precautions should symptoms arise. Patient and father of patient verbalized understanding and agreement. Patient ambulated out of office without difficulty.

Patient received vaccines on 09/28/2022 around 1:00 PM. Dad called and reported to Unit that pateint was having swelling and redness around his eyes with itching. Dad states that he starting complaining about itchy eyes around 09/28/22 around 8-9 oclock that night. Stated that throughout the next days he started having swelling and redness around his eyes. Dad gave him benadryl around 1:30 PM on 09/29/22 and ibuprofen later that night. States that he gave him another does of benadryl this morning on 09/30/2022. Patient continues to have symptoms.

AFTER ADMINISTRATION, HE REJOINED MOTHER AND SAID HE STILL FELT WEIRD AND THAT HE FELT PRESSURE IN THIS THROAT AREA. PATIENT TOLD MOTHER IT FELT PAIN-LIKE FROM THE ADMINISTRATION SITE TO THROAT. ADVISED MOTHER TO HAVE PATIENT SIT FOR ABOUT 15 MINUTES TO MONITOR FOR ALLERGIC REACTIONS. MOTHER ALSO CHECKED PATIENT'S PULSE. I RECHECKED PATIENT AT 15 MINUTES AND HE SAID THAT HE FELT MOSTLY NORMAL AND THAT THE FEELING IN HIS THROAT WAS GONE. TOTAL TIME WAS APPROXIMATELY 20 MINUTES. PATIENT AND MOTHER LEFT TOGETHER.

patient had already received a dose at age 11 and again vaccinated with no adverse event; Initial information received on 23-Sep-2022 regarding an unsolicited valid non-serious case received from a physician. This case involves a 13 years old male patient who experienced patient had already received a dose at age 11 and again vaccinated with no adverse event while receiving vaccine meningococcal A-C-Y-W135 (T CONJ) vaccine [Menquadfi]. The patient's past medical history, medical treatment(s), vaccination(s) and family history were not provided. Concomitant medications included diphtheria vaccine toxoid, pertussis vaccine acellular, tetanus vaccine toxoid (TDAP) and influenza vaccine INACT SPLIT 3V (Fluzone High Dose) both for Immunisation. On an unknown date at age of 11 years was administered meningococcal A-C-Y-W135 (T CONJ) vaccine at an unknown dose (lot details and expiry date: not reported) via unknown route in unknown administration site for immunisation. On 22-Sep-2022, the patient received suspect meningococcal A-C-Y-W135 (T CONJ) vaccine at a dose of 0.5 ml once (lot U7373AA, expiry date: 06-Aug-2024) via intramuscular route in the right arm for immunisation. On 22-Sep-2022 the patient had already received a dose at age 11 and again vaccinated with no adverse event (inappropriate schedule of product administration) (latency: same day). Action taken: Not applicable. Outcome: Unknown. This suspected adverse reaction report is submitted and classified as a medication error solely and exclusively to ensure the marketing authorization holder's compliance with the requirements set out in the Directive 2001/83/EC and Module VI of the Good Pharmacovigilance Practices. On 8/26/2022 patient was due for Pentacel, PCV13, and Rotavirus vaccinations. Vaccination orders were verbally conveyed to medical assistant as well as indicated on patient information sheets per office protocol. Vaccinations logged into patient's chart by medical assistant as Pentacel, PCV13, and Rotavirus on day of visit. Patient received Pentacel and PCV13 as ordered. However,, 8/29/22, it was reported to this physician by the office manager that diluting agent for Rotavirus was in error combined with MenQuadfi vaccine. Diluting agent mixed with MenQuadfi subsequently provided to patient PO. Parents report ~3 days of fussiness s/p vaccinations. Return to baseline health after these ~3 days of fussiness.

MENQUADFI given orally to a 4 month old patient with no adverse event; Initial information received on 29-Aug-2022 regarding an unsolicited valid non-serious case received from a consumer/non-hcp. This case involves a 4 months old male patient to whom menquadfi given orally to a 4 month old patient with no adverse event while receiving vaccine meningococcal A-C-Y-W135 (T conj) vaccine [Menquadfi]. The patient's past medical history, medical treatment(s), vaccination(s) and family history were not provided. Concomitant medications included pneumococcal vaccine conj 13V (CRM197) (Prevnar 13) for Immunisation; and diphtheria vaccine toxoid, HIB vaccine conj (TET TOX), pertussis vaccine acellular 5-component, polio vaccine inact 3V (MRC 5), tetanus vaccine toxoid (Pentacel) for Immunisation. On 26-Aug-2022, the patient received 0.5 mL dose of suspect meningococcal A-C-Y-W135 (T CONJ) vaccine (formulation and strength not reported; lot U7373AA, expiry date: 06-Aug-2022) via oral route for prophylactic vaccination. On 26-Aug-2022, (latency: same day) the patient had a non-serious menquadfi given orally to a 4 month old patient with no adverse event (incorrect route of product administration) following the administration of meningococcal A-C-Y-W135 (T conj) vaccine. It was reported, they have with giving MENQUADFI orally to a 4 month old infant. Action taken was not applicable. At time of reporting, the outcome was Unknown for the event. This suspected adverse reaction report is submitted and classified as a medication error solely and exclusively to ensure the marketing authorization holder's compliance with the requirements set out in the guidelines.
