Immunization nurse was watching client and saw that she was about to pass out. Client had a convulsion of the legs, arched her back and passed out while the immunizations nurse was holding her. Came around very quickly with smelling salts. Patient placed in recliner chair, given juice and crackers.

Patient Blood Pressure taken again at 11:55 am and was 76/49 on the left arm HR 100 Sp02 99. Patient was taken into exam room to lie down and elevate feet. 12:00 pm Patient Blood Pressure 96/64 lying down left arm. Patient sat up and walked out of exam no complaints.

Patient presented to Hospital Center after what appeared to be a seizure /status epilepticus on 9/17. He did have a fever shortly after arrival and had a negative work up for meningitis. He was admitted to the hospital for 5 days. He seems to have fully recovered.

Patient and mother denied history of allergic reaction to food/medications or history of adverse reactions to vaccines in the past. Patient and mother denied health problem. Reviewed screening checklist for contraindications to vaccines for children and teens with patient and mother. After receiving vaccines patient became drowsy. Asked patient if she was ok, patient was able to tell me her name and where she was. As I was going to get help patient loss consciousness on mom. Sitting up O2% was 75%. Laid patient down, elevated her feet. BP: 72/35 P: 58 O2%: 98%, EMS was called. at 1201 BP: 84/52 P: 59 O2% 98%. EMS arrived at 1205. EMS assessed patient and decided to take her to the hospital for further assessment. Mother was by her side the whole time and was kept informed about what was going on. Supervisor was notified.

After applying the band aid following the last immunization (Tdap), Patient fainted in her chair for about 3 seconds. Other nurses were called by CMA and mother. Patient was waking up but confused. Patient was diaphoretic and pale. Blood pressure was monitored as well as O2 and heart rate. Patient was given sips of coca cola and a couple bites of a muffin. Patient started to feel better and was complaining of arm pain. Mom was asked if she would like an ambulance called to transport to the ER and she opted to drive her herself. Patient was helped into a wheelchair and was taken to ER by parents. The next day, we called mom to find out how she was. Mom said the Doctor didn't think it was a vaccine reaction. They thought it was a vasovagal reaction. They gave her the ok to continue getting he vaccinations but to lay down in the future to prevent this from occuring.

She also received her 2nd Trumenba injection the same day in opposite arm (left arm).

After the child received her very last vaccine dose she became pale and unable to follow commands. I assisted her to the ground and elevated her feet and initiated vital signs assessment. Time of vaccination 11:20am and B/P 117/66 Pulse 68. Patient was able to sit up at 11:27 am alert and oriented. She was able to walk out of the clinic with mom without any other assistance.

gave a patient a double dose of MENQUADFI vaccine by accident with no reported adverse event; Initial information received on 28-Oct-2022 regarding an unsolicited valid non-serious case received from a other health professional. This case involves a 16 years old female patient who experienced gave a patient a double dose of menquadfi vaccine by accident with no reported adverse event while receiving vaccine meningococcal a-c-y-w135 (t conj) vaccine [menquadfi]. The patient's past medical history, medical treatment(s), vaccination(s) and family history were not provided. On 24-Oct-2022, the patient received a dose of suspect meningococcal a-c-y-w135 (t conj) vaccine two doses lot U7373AB of ex[iry date 06-Aug-2024 via intramuscular route in the left deltoid for Immunization.. On 24-Oct-2022 the patient developed a non-serious gave a patient a double dose of menquadfi vaccine by accident with no reported adverse event (extra dose administered) following the administration of meningococcal a-c-y-w135 (t conj) vaccine. It was reported "Caller is reporting that they gave a patient a double dose of the MENQUADFI vaccine by accident and they are asking if there are any potential symptoms or adverse reactions they need to be on the lookout for, 16 year old patient was administered two doses of MenQuadfi. She asks for appropriate follow-up. She asks what are the potential adverse reactions". Action taken: not applicable. It was not reported if the patient received a corrective treatment for the event At time of reporting, the outcome was Unknown for the event. This suspected adverse reaction report is submitted and classified as a medication error solely and exclusively to ensure the marketing authorization holder's compliance with the requirements set out in the guidelines. The classification as a medical error is in no way intended, nor should it be interpreted or construed as an allegation or claim made by the marketing authorization holder that any third party has contributed to or is to be held liable for the occurrence of this medication error.
