Systemic: Dizziness / Lightheadness-Medium, Systemic: Fainting / Unresponsive-Medium, Additional Details: Around 1310, I had just exited the clinic and found that patient was sitting in the waiting room chair with mother by her side. Patient looked pale and clammy and was slow to respond. I immediately went to get my gloves, blood pressure cuff, stethoscope, and pulse ox. Pt. had his meningitis injection 3/30/23 2 days later he had neck pain and left shoulder pain He lifted a drum and then he felt a pain like he pulled a muscle and then after that he could not move his neck. He used ice and switched out the pillows and it has not helped It got better and went away for a week and then it came back again. He has pain that is 3/10 right now. He feels a tension that does not allow him to turn his neck.

09:32 Patient reported lightheadedness and dizziness to physician upon standing in room after immunization while getting ready to leave. Dr. advised patient to sit and requested to move patient to room with a chair that leaned back. Patient and father reported patient felt well enough to walk to other exam room. 09:34am Patient requested to stop and use the bathroom, nurse requested father go with patient. Patient turned on light and reported he couldn't see the light, father supported patient, patient's legs went weak. Nursing staff gathered chair for patient to sit. Patient became very pale, diaphoretic, and noted decreased level of consciousness. @09:36 Unable to obtain BP, P - 50, R - 16, O2 sat 99%. Dr. called ER, reported to Dr. who accepted patient. Nursing staff applied cool compress to patient's neck. Patient cooperative, verbally communicating, still appears diaphoretic and pale. Father remains at patient's side. Patient escorted to ER and care turned over. Patient was given oral electrolytes in ER, monitiored, and discharged to home 07/03/23@10:55am.

PT STATES WITHIN A COUPLE HOURS AFTER RECEIVING VACCINE SHE STARTED BECOMING NAUSEOUS, THEN THE NEXT AM NAUSEA CONTINUED WITH SEVERE DIZZINESS. SHE STATES SHE WAS UNABLE TO WALK, AND SHE HAD A PINCHING SENSATION IN HER NECK WITH SHOOTING PAIN. SHE STARTED EXPERIENCING HEADACHES WITH LIGHT SENSITIVITY. PT CONTINUES WITH SYMPTOMS AS OF TODAY. PT DENIES ANY LOC, NO FEVER, NO HX OF MIGRAINES OR HEADACHES. INJECTION SITE HAS NORMAL REDNESS AND TENDERNESS. PT WAS DIRECTED TO ER, BUT MOM CALLED FRIEND WHO IS ER PHYSICIAN AND WAS INSTRUCTED TO HYDRATE AND MONITOR, SO THEY DID NOT GO TO ER.

Symptomtext

I went to get immunzation pt from waiting room and when called mom responded. When brought to exam room it was noted that 2 patients with similar names were both ready to be seen. I had asked mom is this is correct patient and she said yes. Mom was very sleepy during the visit as she had just worked night shift. She kept falling asleep during the visit and one point daughter had to wake her up. I then realized I had wrong patient in my room and had given him the wrong immunizations. Mom was very understanding and knew that there was confusion over the names/patients. Pt received tdap, meningococcal, HPV, and MMR. The first 3 were not supposed to be given at this time. Mom was informed of this and still very understanding. Was present and I also had informed her what happened. They had spoken with mom and explained everything. Appointment was made for child to return on 12-1-22 for dtap and varicella vaccines. I also contacted his school nurse so that they would not put him out of school.
