Pt received TDap along with MCV. MCV was first injection, then MCV. Clinical person stuck the finger for POC hgb, left the room, returned and patient was faint, clammy, c/o stomach was cramping up, stated that he was nauseous. Venipuncture was attempted for labs, that was stopped mid draw. Given emesis basin, water, popsicle in office. Patient was reclined, MD brought in to observe for 15 minutes. After observed, patient was released with father.

Two days after receiving the meningococcal vaccine, my son experienced a hives type of rash circling the injection site on his left deltoid. The area was itchy and white welts swelled up against a red rash on the skin. It started off as a small hive on one side of the arm then it spread all around the area of the injection site within an hour. I did not have an allergy cream so I used cortizone 10 cream on the rash. I applied it the evening it appeared and thankfully it was gone by morning.

16 year old patient was given second dose of MENQUADFI. The first dose was one month ago with no adverse event; Initial information received on 21-Mar-2023 regarding an unsolicited valid non-serious case received from a other health professional. This case involves a 16 years old female patient who was given second dose of MENINGOCOCCAL A-C-Y-W135 (T CONJ) VACCINE [MENQUADFI]. however, the first dose was one month ago with no adverse event. The patient's past medical history, medical treatment(s), vaccination(s) and family history were not provided. Concomitant medications included MENINGOCOCCAL VACCINE B RFHBP/NADA/NHBA OMV (BEXSERO) for Prophylactic vaccination; and INFLUENZA VACCINE (FLUZONE [INFLUENZA VACCINE]) for Prophylactic vaccination. On 21-Mar-2023, the patient received dose 2 (once) of suspect MENINGOCOCCAL A-C-Y-W135 (T CONJ) VACCINE, Solution for injection (lot U7447AB, expiry date: 20-Sep-2024) (unknown strength)via intramuscular route in the left deltoid for immunization. On 21-Mar-2023 reporter reported that 16 year old patient was given second dose of menquadfi. the first dose was one month ago with no adverse event (inappropriate schedule of product administration) (latency: same day). Action taken: not applicable. At time of reporting, the outcome was Unknown for an event. This suspected adverse reaction report is submitted and classified as a medication error solely and exclusively to ensure the marketing authorization holder's compliance with the requirements set out in the guidelines. The classification as a medical error is in no way intended, nor should it be interpreted or construed as an allegation or claim made by the marketing authorization holder that any third party has contributed to or is to be held liable for the occurrence of this medication error.
