Symptomtext

The patient presented to the clinic for a well child check up. He had a history of intermittent dizziness and had two prior syncopal events (Nov 2022, Jan 2023). He received his MCV4 and Men B vaccines and was instructed to sit and wait for 15 minutes (which he did). He then went to the lab facility (inside the clinic) for a fasting blood draw. Per report (Mom), he sat for a few minutes (less than 5), and was then asked if he felt fine and he said he did. He stepped out into the hall and within 1 - 2 minutes, had a pre-syncopal event. Mom states he said "I don't feel well - I need to sit down." He started to spill his coffee. Mom and LVN noted he was pale, his eyes rolled back, and he had bobbing of his torso and head but no tonic-clonic movements noted. He seemed to "crumple" down the wall and sat down. No apparent LOC. No head trauma. When asked, he recalls telling Mom he needed to sit down and then about 1 minute memory lapse. He was carefully moved to an exam room and observed with repeat vital measurements (all were normal). In clinic (before blood draw) Orthostatic P/BP Pulse Lying: 61 PER MINUTE BP Lying: 103/64 Pulse Sitting: 79 PER MINUTE BP Sitting: 111/77 Pulse Standing: 80 PER MINUTE BP Standing: 102/73

Dizziness Loss of consciousness Pallor

Symptomtext

After HPV (third vaccine given during visit) patient became pale and stated she felt like she was going to "pass out". Patient lost consciousness for 30 seconds, nurses provided ammonia inhalant and patient regained consciousness and was monitored for 15 minutes after event before she was released.

Feeling abnormal Nausea Presyncope Syncope

Symptomtext

Pt was sitting on the exam table, Initial BP taken 129/86 left arm sitting, pt did express that she was very anxious. This nurse reviewed possible side effects of the vaccines that are going to be administered, Pt and mother both verbalized understanding of education provided. MCV4 administered in the left arm, pt tolerated procedure. HPV given in left arm, pt appeared to be holding her breath, pt appeared to be having the beginnings of pre-syncopal episodes. This nurse placed her hands on the pt back and arm to provided support at 419 pm pt appears to have fainted, this nurse called out for help Dr and NP, and multiple nurses responded. pt was then laid down on the exam table. Pt did not fall or sustained any injuries. pt became alert at 420, startled awake and trying to sit up, this nurse reassured the pt and had her lay down on the exam table. 421 PM B/P L arm supine 157/54 HR 90, 424 PM B/P L arm supine 119/78 HR 88, 428 PM B/P L arm supine 117/74 HR 82, 436 PM B/P L arm supine 122/77 HR 81, 442 PM B/P L arm sitting 117/76 HR 78 , 448 PM B/P L arm standing 119/80 HR 80. Once pt was alert she complained of feeling funny, and nausea. Pt attempted to sit up at 430 she drank a few sips of water and stated that she didn't feel good and laid back down. At 440 pt stated that she felt better and wanted to sit up, Pt sat on the side of the exam table and drank a glass of water. Pt stated at this time she is feeling better. At 448 pt was able to stand unassisted, with no complaints of dizziness, blurred vision, nausea, or SOB. Reviewed with the pt and mother on syncope episodes and future medical procedures/vaccines both verbalized understanding. Dr. escorted pt to their car, pt was able to ambulate unassisted to the car. Follow up with pts mother on may 10 2023 , no complaints states" she is doing fine.", pt went to school that day.

patient felt hot/sweaty and dizzy then passed out for ~5 seconds. She slumped over, did not fall. No resulting injury sustained.

Symptomtext

Medical Assistant had performed chart prep and indicated that the patient was due for his 2nd Menquadfi. A different MA gave the vaccine and later found that the vaccine record had not been uploaded into the chart and the patient had already had his 2 Menquafi. Patients' dad was made aware.
