Symptomtext

The patient told me he had an acute fear of needles and vaccines. He was very nervous and shaking. He was seated, I told him to look at his mom and concentrate on her face. I administered the vaccine around 1.09pm while telling him he's doing a good job. He appeared normal afterwards, and he and his mom proceeded to walk down the corridor in front the pharmacy. He told his mom about a minute after that he felt faint, and fell on her shoulder, then unto the ground. He hit his head. He fainted. He was conscious a couple seconds after and talking normally. I gave him a bottle of water and orange juice, which he drank. He did not eat breakfast that morning and was leaving for college in 2 days and felt overwhelmed. His head was sore from the fall. He said he felt fine after 10 minutes. He left the store on a powered cart to go home.

Symptomtext

Patient was nervous prior to getting vaccines, patient was distracting herself by being on her phone while I was about to inject vaccines. Injected MenQuadfi and patient was ok and then I injected Bexsero, and patient was still doing ok. She did complain about the vaccines hurting as they were administered. Did a Tuberculosis testing on patient as well, as i removed needle after administrating it, patient started to slowly lean and collapsed to the floor. Patient was picked up and sat down and was crying and was feeling pain in her face, since she collapsed face down. Nurse came in to see how patient was feeling, patient stated she was ok and that she only felt pain on her face. We asked her if she was feeling dizzy or nauseous or having an head pain. Patient stated no. She was only confused on why she had collapsed she stated that all she remembered was hitting the floor. Patient was kept in the facility for a few more minutes to make sure she was ok to leave home. Patient was advised to go to the ER if she experiences nausea, dizziness and headaches.

Diarrhoea Fatigue Nausea Vomiting

Symptomtext

vaccine given 8/3/23 in the morning. On evening of 8/3/23 patient developed nausea with vomiting. On 8/4/23 patient had fatigue, nausea anal diarrhea. He also experienced this on 8/5/23. Symptoms resolved by 8/6/23.

Hypoaesthesia Tinnitus

Symptomtext

Patient received Tdap, Meningococcal Conjugate Quadrivalent (Left Deltoid) and HPV (Right Deltoid) vaccines today during well visit. Upon completion of vaccines, after leaving room and walking to check out, patient stated to parent that he could not feel his hands and heard "noises". Parent escorted patient to a seated position in chair while Patient Service Specialist notified Registered Nurses. RNs immediately responded. Patient instructed to lay down on parent, was pale at this time. Patient provided with apple juice, ice water and a cool wash cloth for forehead. Patient's skin color began to return and stated that he could feel his arms again after resting and having some juice. Stated that he no longer was hearing noises. Patient was then instructed to sit up into a seated position to see how he tolerated. Reported that he felt better. After approximately 5 minutes, was instructed to stand. Upon standing reported that he felt fine. Patient then remained seated in lobby for an additional 10 minutes prior to leaving. Reported at check out while stating that he was better.
