Syncope

Symptomtext

After patient received third vaccine, patient fainted. RN was able to keep patient on exam table during fainting episode. After 15 seconds, patient became A&O x5. Patient was given juice and sugary snack. Patient's blood pressure at 11:14am, 2 minutes after fainting episode was 114/69. Patient remained in office for 15 minutes for observation post-fainting episode. Patient's blood pressure at 11:30am, prior to patient leaving, was 124/67. Pt stated she felt well and stated that she was able to stand. Pt walked out of clinic with sister without difficulty.

From ER note: Initial chief complaint was Hives (Meningococcal vaccine on Friday. Saturday "vision just blacked out but still conscious", hands "felt hot". Vomiting and fainting on Sunday, hives appeared to inner arms. Discoloration to arms and legs "they looked purple but then it went away", hives spread to arms and legs now. 15mL of Bendryl at 1315 today for allergies. Hx of allergies of hives and edema as reaction to cold, no allergy symptoms for 4 months until last 3 days. Pediatrician referred to ER for possible infection.), Fainting, Skin problem, and Vomiting Patient is a 13 year old biological male who presents with hives. Three days ago, the patient received meningococcal vaccine. Since then, he is had 2 episodes of presyncope where he has felt lightheaded and like there was a heaviness in his extremities; he has never actually lost consciousness however. Onset after shower. Two days ago, he reports single episode where he vomited 3 times; he is otherwise had no vomiting besides this. He also denies any diarrhea or any bleeding. No palpitations. No family history of sudden cardiac death or cardiac issues at early age. Today, while walking in the emergency department, he had a transient episode where he developed some blue discoloration on his right calf that has since resolved. Symptoms resolved. He otherwise denies any recent fevers, chills, or neck pain. Of note, he did have brief contact in the last week with someone who is being treated in the hospital for meningococcus. History of angioedema and hives with cold exposure and hand swelling with minor trauma.

Systemic: Fainting / Unresponsive-Medium, Systemic: Seizure-Medium, Additional Details: 10:28 - Following administration of vaccine, patient while still sitting in the chair had an episode of syncope with seizure-like activities which lasted for 10 secs. 10:30 - Patient regained consciousness at which point EMS was activated and call placed to patient's mother to notify her of the situation. Symptomtext

Complains of a sore left arm since the injections on 7/5/23. She felt that the vaccines were given to high and hurt more going in and afterwards than usual. She has had pain the whole time since. It started to get better but has plateaued since. She cannot use full range of motion of her shoulder due to the pain. It isn't red, hot or swollen and there is no fever.

Symptomtext

Mother called our clinic 7/25/23 and reported that her daughter received Immunizations at her check up 7/11/23 and the injection she received in left arm caused her to have limited range of motion in shoulder joint. Could not lift left arm up high making combing her hair and other activities difficult, couldn't lay flat on back without putting something under left shoulder. Tenderness to left shoulder joint. Mother says in last few days has had some pain up into left side of neck. Mother took her to a urgent care 7/15/23 was prescribed Tizanidine 2mg po q8hr prn. Mother was told it was a sore muscle and inflammation. Mother took patient to Chiropractor 7/25/23. Mother called our clinic for first time to inform us of the issue her daughter was having 7/25/23 and requested a referral be done. Patient set up for follow-up and referral 7/26/23.

Symptomtext

Aching feeling in left arm at the site, joints hurting, headache, blurred vision, and body aches. Symptoms occurred on 8-21-2023. He stayed out of school on 8-22-2023 and returned to school on 8-23-2023. The parent stated her son was OK to return to school on 8-23-2023. The client's mother said she contacted office and was instructed to give her child Tylenol and Motrin and alternate.

Symptomtext

Patient became dizzy and nauseated after vaccination. Patient c/o not being able to see clearly. Feet propped in chair. Cool air and cool paper towels placed on neck and face. Patient vomited approximately 50-100 ccs. Stated that she felt better after vomiting. Coke and crackers given. After approximately 10 mins patient felt better and was able to ambulate to lobby with dad. Dad is a physician assistant. Advised dad to wait in lobby an additional 10-15 mins before they leave. Dad voiced understanding. Copy of immunization records given. Reviewed possible side effects of immunization(s). Advised for patient to wait in lobby for 20 mins after shot(s) to monitor for reaction.

pregnant received menquadfi with no reported adverse event; Initial information received on 12-Jun-2023 regarding an unsolicited valid non-serious case received from a pharmacist. This case involves a 18 years old female patient who was exposed to vaccine meningococcal A-C-Y-W135 (T conj) vaccine [Menquadfi] in context of pregnant with no reported adverse event. Data regarding this pregnancy were received prospectively. The date of last menstrual period was Jan-2023. The estimated due date was not reported. The patient's past medical history, medical treatment(s), vaccination(s) and family history were not provided. On 12-Jun-2023, the pregnant patient received 0.5 mL dose 1 of suspect meningococcal A-C-Y-W135 (T CONJ) vaccine (batch number: U7616AC; expiry date: 11-Apr-2025) via intramuscular route in left deltoid for Immunization with no reported adverse event (exposure during pregnancy) (same day latency). Action taken: not applicable. At time of reporting, the outcome was Unknown for the event pregnant received menquadfi with no reported adverse event.
