Syncope

Symptomtext

Patient had a syncopal episode approximately 5 minutes after administration of the vaccines above. The patient was assisted to a lying position, aroused with verbal and tactile stimuli after 3-5 seconds. The patient was anxious prior to vaccine administration, per Mother it had been many years she had immunizations. Cool compress applied to forehead and the patient was given water and cookies, BP: 100/80 Pulse: 88 Resp: 26. The patient was monitored for approximately 15 minutes, till she verbalized she was better. Mother refused EMS transport to ER for evaluation.

Symptomtext

Upon administration of second vaccine, HPV, patient stated he was lightheaded and had a syncopal episode. Assisted patient on laying on bed, patient was alert within seconds and instructed to lay with knees bent. Patient sat up within 5 minutes and given water. Patient was observed by mother and nurses. Patient stated he wanted to go home and it was determined that he was able to ambulate on his own. He was accompanied outside walking by mother. Patient's color was back to normal and was alert and oriented.

Dizziness Seizure Syncope

Symptomtext

Patient complained of dizziness and was offered water to sip while sitting in a chair with arms and a healthcare professional immediately in front of the patient. Patient then fainted, started to seize, then quickly regained consciousness. Had patient lie down and elevated her legs. Patient then started shivering and so patient was covered with a blanket. After laying down for approximately 10 minutes, patient stated she was feeling good enough to sit up. Patient sat up for another 5 minutes and stated she felt normal. Patient waited another 5 minutes with her mother and recovered without sequelae. Patient left under the care of her mother.

Symptomtext

Patient complained of not being being able to catch his breath. Chest was feeling tight. No complaints of feeling dizzy. 10:35 am his vitals 02 sat were 99 Heart rate was 85 Blood Pressure 100/65 respiration 24 and patient stated he felt better. NO signs or symptoms of respiratory distress noted. Vital signs prior to release at 10:50 am were 02 sat 99%. Heart rate 82. Blood pressure 108/66. Respiratory rate was 20. Patient stated he continued to feel fine. No respiratory distress noted. Released to leave at 10:53 with guardian/aunt.

Symptomtext

About 30minutes after injection and taking 400mg of ibuprofen, he returned to office with angioedema and hives on upper eyelids bilaterally. No shortness of breath, no dizziness no nausea, no hives elsewhere on body. 10mg of Loratidine was given and he improved.

Symptomtext

Patient was vaccinated at 1:05 with Hep B, at 1:06 with MCV4, at 1:07 pm with varicella. At 1:08, Patient stated, "I feel dizzy". Patient became sweaty. Applied cold pack to the back of patients neck. BP 100/61, O2 sat. 99% RA, P 68, Resp. 20, At 1:15 patient was alert and oriented x3, stopped sweating and stated, "I feel better". BP 110/78, P 64, O2sat 99%, Resp. 20, At 1:35, patient had no symptoms of distress or dizziness. Patient smiling and stated, "I feel fine". BP 114/78, P 72, Resp. 20, Temp. 97.7, O2 sat 99%. Alert and oriented x 3. Discharged with father and instructed to call EMS or ER if symptoms return. Patient verbalized understanding.

PATIENT GOT LIGHTHEADED AND PALE. PARENTS SAID SHE HAD NOT EATEN BREAKFAST. SHE DID NOT FALL DOWN. SHE SAT DOWN NEXT TO PARENTS. SHE DID NOT LOSE CONSCIOUSNESS. NURSE APPLIED COLD PACK TO BACK OF HEAD AND GAVE HER A LOLLIPOP TO EAT. FANNED PATIENT AND ASKED HER ORIENTATING QUESTIONS. AFTER 10 MINUTES HER COLOR TO FACE WAS BETTER THAN BEFORE AND STATED FEELING BETTER. WALKED UNASSISTED OUT OF THE CLINIC WITH PARENTS.

Cold sweat Headache Pallor Thirst

Symptomtext

Patient with grandmother to receive vaccines. After administration of vaccines at 9:55am, patient stated having a headache. Nurse noted pale and clammy skin, and vitals were taken at9:56am with BP100/66 , HR 62, 0xygen saturation at 100 %. Patient then stated feeling thirsty at 9:57am, water was provided and AAOx4. Vital signs were taken at 10:03am, BP 106/84, HR 80, oxygen saturation at 100%. Patent was able to continue communicating and verifying name and date of birth . Patient stated , " im starting to feel better now". VS taken at 10:08am with HR at 77, Oxygen saturation at 100 %, BP 104/80 with signs and symptoms of improvement. At 10:15 patient reported not feeling lightheaded, headache had diminished, and showed no signs of distress. Patient and mother were advised if symptoms were to return , to seek emergency assistance. Both patient and mother verbalized understanding. Patient was able to stand up slowly without assistance and was released from our care.

This case involves 11 years old male patient who reported that previously reported temperature excursion for menquadfi with no reported adverse event and after temperature excursion clinic did administer a dose of the menquadfi to a patient with no reported adverse event after receiving meningococcal a-c-y-w135 (t conj) vaccine [Menquadfi]. The patient's past medical history, medical treatment(s), vaccination(s) and family history were not provided. On 15-Sep-2023, the patient received a dose 1 of 0.5 ml of suspect meningococcal a-c-y-w135 (t conj) vaccine (Solution for injection) (lot U7633AB, Expiry date :25-Apr-2025, strength was unknown) via intramuscular route in the right deltoid for Immunization. On 15-Sep-2023 the patient previously reported temperature excursion for menquadfi with no reported adverse event (product storage error) and after temperature excursion clinic did administer a dose of the menquadfi to a patient with no reported adverse event (poor quality product administered) (latency : same day) following the administration of meningococcal a-c-y-w135 (t conj) vaccine. Action taken with meningococcal vaccine (Menquadfi) was not applicable. Fall Head injury

Symptomtext

Patient received initial dose of HPV, TDaP and Menquadfi. It was reported by vaccine administrator, CMA, that shortly after administration of HPV (which was the last vaccine administered), patient smiled at the administrator and proceeded to fall forward from exam table. CMA attempted to hold patient up while falling down but did manage to hit forehead on linoleum flooring. Mom called for help immediately and physician went into exam room to assess patient and was kept in clinic for additional hour for observation. Vitals were stable and patient and oriented when dismissed home with mother.

Patient received vaccines at the Mobile Site. Patient stated after receiving the vaccines that she felt hot. Patient skin pale in color. Patient mother stated patient had not eaten. Patient mother feed patient Jello and a apple. Patient given water to drink. Patient color return to appropriate color for race. Patient denied feeling hot. Patient Temperature- 98.4, Blood Pressure-100/62, SATO2-98 %, Respirations-16, and Pulse-91. Offered to call emergency services for patient. Patient mother and patient refuse emergency services. Patient monitored for 30 minutes. Patient and patient mother denies any complaints prior to leaving post vaccination area.

Vaccination error

Symptomtext

Rn went to administer vaccinations. Orders reviewed and mistakenly thought Menquadfi was ordered. Pulled form vaccine fridge and prepared by this RN. Administered to child after verifying name and date of birth as well as vaccines receiving. Aunt in room with child. Child tolerated well. Patient left. RN charting vaccines approximately 30 minutes later when downtime available. RN switched to vaccinations tab to chart vaccines. Noticed Menquadfi not ordered. Verified error by viewing vaccination record. Notified promptly.

he was scheduled to receive a tdap, administering nurse gave him another meningococcal vaccine, which is confirmed to be MENQUADFI with no reported adverse event; menquadfi dose administered too early given 8 months apart with no reported adverse event; Initial information received on 12-Jul-2023 regarding an unsolicited valid non-serious case received from a consumer. This case involves a 12 years old male patient who was scheduled to receive a tdap, administering nurse gave him another meningococcal vaccine, which is confirmed to be meningococcal A-C-Y-W135 (T CONJ) vaccine [Menquadfi] with no reported adverse event and menquadfi dose administered too early given 8 months apart with no reported adverse event. The patient's past vaccination(s) included meningococcal vaccine on 17-Nov-2022. The patient's past medical history, medical treatment(s) and family history were not provided. On 11-Jul-2023, the patient received an unknown dose of suspect meningococcal A-C-Y-W135 (T CONJ) vaccine, solution for injection (batch number: U7633AB) (expiry date unknown) via intramuscular route in the left deltoid for immunization while he was scheduled to receive a tdap and dose administered too early given 8 months apart with no reported adverse event (wrong product administered) (inappropriate schedule of product administration) (same day latency). Action taken: not applicable. At time of reporting, the outcome was Unknown for both the events. This suspected adverse reaction report is submitted and classified as a medication error solely and exclusively to ensure the marketing authorization holder's compliance with the requirements set out in the guidelines. The classification as a medical error is in no way intended, nor should it be interpreted or construed as an allegation or claim made by the marketing authorization holder that any third party has contributed to or is to be held liable for the occurrence of this medication error.

I was a vaccinator at clinic. I requested Hep A, MCV4, and Monkeypox for an adult patient. I was given a prefilled syringe of Hep A, a vial of MCV4, and MPX by the vaccine manager. I was unaware I was given state vaccines and administered vaccine to patient. It was later discovered the vaccine were state and the patient was under vaccinated. Corrective Actions is being taken.

Symptomtext

Patient came into the clinic for family planning services. Patient had not missed a period and wasn't due for a period for a few more weeks. Menquadfi was due and was administered before the patient was seen by the practitioner. Once the patient was seen by the practitioner, the practitioner ordered a pregnancy test because she was going to place a Nexplanon in the patient. The pregnancy test came back positive. Patient's last period was June 2, 2023 and menquadfi was administered June 27, 2023. Patient tolerated the vaccine well. Patient's due date is determined to be 3/8/24.
