Dysstasia Hyperhidrosis Syncope Vision blurred

Symptomtext

Client and family were exiting the facility when the client became unsteady on his feet and had an episode of syncope. Family helped client to the ground. Client had received 3 vaccines and a TB skin test. We assessed client and father helped client to an upright position and placed him in a chair. Client was diaphoretic. We gave him 2 bottles of juice. Vitals signs were taken at 2:19pm: BP75/30, HR 65. (per machine). BP rechecked manually at 2:25pm and was 110/56. Client did c/o blurry vision at the time he had the syncope episode. Family refused ambulance service. Client recovered and went home with family. Family given instructions to take BP every hour and drink lots of water by nurse practitioner and if in doubt take client to urgent care or ER for follow up.

Symptomtext

Client stated feeling lightheaded after vaccination. Client parent stated client had not eaten. Client lowered to the floor, legs placed on chair, and cool towel placed on client head by LVN. Client stated no longer feeling lightheaded. Client sat in chair. Client vital signs checked. Client blood pressure- 104/62, temperature- 97.3, pulse- 62, oxygen saturation- 98%, and respiration- 20. Client denies having any other symptoms prior to leaving post vaccination. Client parent stated that that they were going to eat. Client denies any distress prior to leaving post vaccination area.

Symptomtext

Patient experienced localized edema and redness around injection site. This presented the following day. Started small and then grew. At time of report the edema is at 7 cm by 8 cm. Patient did not present nor report and rapid heart beating and or shortness of breath.
