Symptomtext

ADVERSE REACTIONS: Redness on right deltoid after Gardasil administered. PT felt soreness on both arms after all 3 vaccines administered. PT passed out for about 15-20 seconds. PT felt hot, sweaty, weak and nauseous TREATMENT: PT had cool packs to the neck, sipped water slowly, sat down w/ feet elevated above heart and gave crackers to eat. OUTCOME: PT no longer had redness to the vaccination sites, Pt no longer felt soressness, nauseous, hot, sweaty, and weak. PT bp was normal range -1st assessment bp - 100/70 @ 1400 -2nd assessment bp - 103/72 @1405 -3rd assessment bp - 103/72 @1410 PT left between 14:20 - 14:30

patient felt weak after shot and sat down. was pale and on walking back to exam room, slumped to floor (gently lowered by staff). awake and interactive upon lying down. recovered completely after 30 minutes. had diaphoresis. no diff breathing. had not eating for 9 hours before shot. able to eat and drink. vitals stable. event c/w vasovagal episode

Symptomtext

Patient had a seizure shortly after receiving vaccines. Patient had been very anxious prior to receiving her vaccines. The patient had a seizure and was placed on the floor on her side. EMS was called, vitals obtained, patient monitored. Patient transported by local EMS to local hospital. Guardian said "It's not the vaccines, she has a seizure disorder".

Patient complaining of headache, dizziness, fatigue, 10/10 right arm pain. Patient here to see provider in regards to vaccine given yesterday 10/17/23. Ibuprofen ordered for pain control. Patient has a follow up appointment in 1 week to follow up. Patient instructed to come see the provider if not any better or if pain get worse. If any signs or symptoms of infection noted.

for two days, has redness, swelling from both left and right arm where injections where given. It is warm to the touch, admits has been scratching at site of injections. Patient was seen today on 10/06/2023 and was prescribed medication which mother will start giving to patient. Called today 10/09/2023 to follow up and there was no answer.
