Symptomtext

Flu vaccine was given first. No immediate reactions. Then 1-2 minutes later, the MenACWY was given and almost immediately, pt c/o blurry vision. About 3 minutes later she had difficulty breathing, almost apneic and passed out (never lost a pulse).- anaphylactic reaction. 911 was called while an Epipen was immediately and simultaneously given. About 1.5 minutes later, she gained consciousness and began to breathe normally again. Vital signs were stable. 911 appeared and transported to the hospital.

Decorticate posture Eye movement disorder Malaise Seizure

Symptomtext

Patient was present with dad while vaccine administration, after receiving 7 out of the 9 vaccines, patient reports not feeling well, and experienced seizure with decorticate posturing manifested by eyes rolling upward, legs becoming straight and and arms flexing upward with clenched fists holding close to chest. I had the patient lay down flat on the exam table with head turned to the side. Seizure lasted approximately 5 seconds, and afterwards patient became alert and responsive. Father was at patient's side. V/S were checked. BP 122/73, HR 94, RR 20, Temp 98.1. Patient recovered right away as soon as she lay down on the exam table. Patient was due to also get Hep B and HPV vaccines, which was held d/t this event. Father was advised to F/U with patient's PCP for further vaccine administration.

Nausea Nervousness Pallor Syncope Unresponsive to stimuli Vomiting

Symptomtext

Patient in office with mother to receive vaccines. Mother accepted offer for Tdap, HPV, Meningitis. Patient given Tdap and Meningitis and did well. After RN injected HPV vaccine around 4:15pm patient face turned pale, and he then fainted. His arms stiffened as he was fainting, and he was then unresponsive for about 20-30 seconds. RN laid patient back on table during event and called for help. Additional nurse responded and helped RN to raise patient?s legs and place cold rag on patient forehead. Patient then came to and was able to tell nurse his name and explain that he was feeling shaky. Initial blood pressure about 4:20pm BP 109/89 HR 89. Patient given juice due to fact that child states that he has not eaten all day. BP check 4:30pm 90/66. Patient stated after drinking juice that he felt nauseous and vomited. BP check 4:40 89/59 HR 74. Patient reported feeling better although his face was still pale. Mother decided to take child home. RN advised mother if patient fainted again, she should take him to ER. Mother voiced understanding. Patient walked down the hall with mother and then sat in waiting room until checked out. Mother walk out the door with child in stable condition.

Symptomtext

Vasovagal reaction to vaccine, patient had a near fainting experience, a few seconds after receiving her vaccine. We had the patient lie supine on the table and drink juice. The patient stated she felt better so we had her sit up on the table, she then felt faint again and her skin was pale and clammy, we had her lie back down on the table and continued to give her fluids of juice and water while monitoring her blood pressure and pulse. Patient was monitored for 1.5 hours and given 50 oz of water to drink while in the office. Patient was released to leave our office after her vital signs stabled and she was feeling better.

Symptomtext

Pt received vaccine on 7/5/23. She began having rhinorrhea around 7/10 or 7/12 so parents started treating with cetirizine & benadryl. Had a headache and possibly vomiting around 7/9. She began having a circumoral rash around 7/15 or 7/18 and parents went to urgent care on 7/21, was prescribed bactrim for the rash, and had two doses that night and the morning of 7/22 at about 10 am. She began having a blotchy rash on the hands around 11 am so parents presented to the emergency department thinking there was a reaction to the bactrim. From the time they left their house, patient was able to walk with slightly ataxic gait, and upon presentation to nearby ER (within approx 15 minutes), patient was unable to walk at all. She continued to have rapid ascending paralysis up to the upper arms. In the ER patient did have vomiting with the hand rash and was treated for anaphylaxis (benadryl, intramuscular epinephrine). Admitted to PICU and received IVIG x2 doses and now on high dose steroids. Has also been on antibiotics since 7/22 - 7/25 at time of filling the form. Developed fevers 2 days after IVIG treatments and has thick secretions/congestion. Her neurologic exam from 7/22 - 7/25 has stayed stable (complete weakness of lower extremities without any movement, no reflexes of lower extremities or upper extremities, and poor coordination of upper arms but can flex biceps and very subtly lift arm off the bed).

CSF 7/22: 1 WBC, normal protein, glucose 83, meningitis / encephalitis panel negative 7/22: Plasma cell profile with beta 0.5, gamma 2.0, albumin 3.2, normal ferritin, ceruloplasmin, LDH, normal iron studies, normal anti-thyroglobulin & TPO Ab 7/24 CMP BUN/Cr ratio 27, albumin 2.7, globulin 5.2, A/G ratio serum - 0.5 7/25 CMP: Alk phose 103, albumin 2.4, globulin 4.9, serum A/G ratio 0.5, glucose 145, Cr 0.35, BUN/Cr ratio 37 7/25: procalcitonin 2.04 CBC 7/24 Hgb 9.4, CBC 7/25 8.9 CRP 7/25 0.3 (negative) Urinalysis 7/25 - few bacteria, otherwise normal Ab lyme screen on blood (7/23) - negative Blood culture 7/23 - no growth x2 days

Symptomtext

Patient complained that he felt dizzy and difficulty breathing after 10 minutes in post vac. Patient instructed to deep breath, cold compress and ambulance called, as patient anxiety increased. EMS took over. VS WNL. Patient was not transported to hospital. Parents declined.

Symptomtext

Tremors, vomiting, shortness of breath & dizziness within 30 minutes of administration. Vaccines administered around 9:11am on 07/24/23. Pt was brought back into the office on 07/24/2023 at 1:50pm - (3) sets of vitals completed around 2:00pm which were: 110/71 w/ HR of 94. 118/74 w/ HR of 87. 114/68 w/ HR of 76 & SPO2 of 97 on room air. EMS was contacted and pt was transported to ED

Fatigue Headache Nausea Pain Rash

Symptomtext

Pt woke complaining of headache, body ache (particularly his arms), fatigue and severe nausea. He did not vomit. Sxs lasted 2 days and were followed on the 3rd day by a patchy, hive-like rash (without raised welts) on the whole upper right arm (MCV side). No swelling. Rash lasted 2 more days, then self resolved.

Symptomtext

Patient has developed an itchy rash around one of the vaccines place on right arm (had influenza and MCV) on that arm. Not sure which one. Mother place cool towels and used benadryl cream, but still itchy. No getting worse. No systemic reactions

On 09/05/2023, one dose of Pfizer-BioNtech Covid-19 vaccine, Bivalent Ages 5 to <12y , 0.2mL was administered intramuscularly to the patient who turned 12 years old on 08/30/2023. Patient did not exhibit signs and symptoms of an adverse reaction.
