Dizziness Head injury Syncope

Symptomtext

After vaccination, patient waited in the lobby with dad and brother for about 5 minutes. When they got on the elevator, patient states she started to feel dizzy and fainted, hitting her head on the way down. Dad quickly hit the button to open the elevator since it had not started to go down yet. They came to one of the clinic rooms and patient was told to lay down. Patients BP was 87/51, Pulse 70, R 18, Temp 98.7, and was pale. We gave her a granola bar and water. After about 5 minutes, patient stated feeling better and color started returning to face. Denies nausea or any additional symptoms. At 3:15pm her BP was 108/77. At 3:25pm, her BP was 113/64. Patient remained feeling well after sitting up on the cot. Explained to dad it is his choice to go to FMD or ER to have treatment from fall. Discussed symptoms to be aware of in regards to head injury. Dad states understanding. Denies additional questions/concerns. Patient ambulates to car with no issues.

Fall Loss of consciousness

Symptomtext

Patient received HPV and MenQuadfi vaccine, pt was in waiting room when this nurse was notified that patient fell into mothers' arms in waiting room. Patient regained consciousness, was then brought into an exam room in where patient was placed in supine position with legs elevated. Vitals taken and recorded; MD notified. Patient was given juice, water, and ice pack. Patient felt much better, was monitored for 10-15 minutes. MD assessed patient. Patient observed for another 10 minutes. Patient was able to sit up slowly and then 5 minutes later proceeded to stand up slowly and walk around. Patient then left with parent who felt comfortable and safe monitoring child outside of the office.

Hyperhidrosis Seizure

Symptomtext

vaccines completed at 1225. Approx 1230, mother yelled and pt witnessing having seizure activity resembling a grand mal seizure. Seizure activity lasted approx 7-8 seconds. Pt was not incontinent. Pt diaphoretic. Pt looked over at mother post activity and asked what happened. Pt stated she felt like she had been in a long dream. vitals taken several times and stable. Pt sees neuro for seizure activity--mother states she has frequent focal seizures but has not had grand mal activity for several /years. Called and talked to mother on 08/15/2023 and she stated pt has had zero seizure activity since and is awaiting guidance from neuro doctor of plan of care.

Symptomtext

Patient was fine in the clinic and waited 15 minutes before leaving clinic after she received the injection. Caregiver called the clinic at 10:19 AM and stated her daughter was having a severe Headache. She stated she was crying stating her head hurt so bad. Caregiver gave child 3 ibuprofen and then was going to take the child to the MD office and she stated that her daughter the patient turned white and was very slow to respond. She went to the ER and said upon admission her B/P was 124/46.

All the ER nurse and the caregiver stated was that blood work was collected along with a urine sample and the MD stated all the results were normal. They also stated that she received on 1000ml of Normal Saline per IV. Caregiver stated the patient was discharged at approx. 1:15PM. She stated the patient still complained of a H/A, but that it was much better.

Dizziness Nausea Nervousness Pallor

Symptomtext

11 year old female patient came in with parents for school vaccines. She received 5 vaccines (tdap. flu, MCV4, varicella and polio). Immediately after being vaccinated patient became nervous, hot, paleness to skin, dizziness and some nausea. Vitals was taken and a wet cold cloth was put behind her neck. Pt. drank small sips of water and after a few minutes she started to get color back to her skin. She then open her eyes, raised her head. Recovered in 5 minutes.

Dizziness Dyskinesia Nausea Pallor

Symptomtext

Patient was screened for contraindications, and had no true contraindications. Patient and Mother was advised to stay for 15 minutes for observation after receiving vaccines. Parent Verbalized understanding. Meningococcal (MCV-4) IM and Tdap (adol/adult formulation) IM, and HPV (Human Papilloma Virus) IM given per protocol using proper technique. At 1124 after receiving the HPV vaccine, patient began to clench her fists bringing her arms closer to her body that lasted 4-5 seconds as she looked up and responded, "what happened?, am I done getting vaccines?". Instructed patient to take deep breaths. No lip biting and grinding of the teeth was noted. I immediately obtained vital signs while she remained seated while also asking her for her name and date of birth, location and who did she come with. Patient was oriented to place and person. First vital signs were obtained at 1125a BP 69/48 HR 55 O2 saturation at 95% skin complexion was pale with no sweating noted while patient was asked if anything was wrong, and stated, "i feel dizzy and nauseated". Mother stated patient had no prior history of vaccine reaction and reported no allergies at this time, and patient had nothing to eat this morning. Patient remained conscious, Second set of Vitals signs were obtained at 1130a BP 82/52, HR 62 O2 saturation at 96%. Third set of vitals obtained at 1135 BP 99/65 HR 68 O2 saturation at 98%. Skin color returned with no signs of discoloration noted. Patient remained conscious and talkative. Vital signs obtained at 11:43a BP 103/70, HR 74 O2 saturation at 98%.. Patient was stable and was able to stand without assistance while I remained next to her, and was able to walk with no assistance. Parent and patient were educated to call for emergency care if symptoms return or worsen. Parent and patient verbalized understanding and released from my care our care at 11:48a Provider was notified and VAERS report submitted

Symptomtext

Approximately 3-5 mins after administration the patient stated he felt like he was going to pass out. He became dizzy, sweaty and stated he couldn't hear as well as before getting the vaccine. Pt remained seated the entire time and never loss consciousness. Vitals at 1835: 02=100% BP=107/53 RR=18 HR=53 At approximately 1840 he stated feeling better and his symptoms resolved completely a few mins afterward. Vitals at 1840: 02= 100% BP=117/72 RR=20 HR=57. Pt stood up slowly after last set of vitals were taken and left the vaccine site at approximately 1845 with no signs or symptoms of distress or complication from the reaction.

Pt was given meningococcal vaccine that did not need the vaccine at this time. Noted that sibling was at same appointment and both received the vaccine. This patient had received Menactra at age 11 (11/20/19) and would not have been due for booster for another year - vaccine administered 8/9/23 was given a year too early.

Symptomtext

Pt received a Meningococcal vaccine that was not actually needed at this time. Review of chart indicates patient has been off schedule with meningococcal vaccines for the past several years. 8/22/2017 received Meningococcal MCV4 per health department. 7/22/2021 received Menactra 7/25/2022 received Bexsero 8/9/2023 received this vaccine reported- Menquadfi. Noted that patient is of age to start collage and may have been told they required some form of the vaccine to start school. No adverse effects.

Symptomtext

Patient left exam room after receiving MenQuadfi vaccine and walked into waiting room to check out. There, a receptionist noticed patient was pale and called in two nurses from the office who responded to the event. Nurses obtained vitals and notified patient's provider, Dr, who suggested patient lie down on exam table with feet on stool and continue assessment of vital signs, vital signs stable. Patient remained in office for at least 15min, vital signs remained stable, Mom verbalized she felt comfortable taking patient home, Nurses advised parent to call office with any questions or concerns.
