Symptomtext

Approximately 10-15 minutes following above vaccinations (at 9:55 am), patient began to have hives at the injection site where the Yellow Fever vaccine was administered. There were approximately 3 hives within 1-2 inches from injection site. She then began to feel some dry sensation in her throat and was transferred to Urgent Care at 10:00am (located in same building at medical facility). Began to report itching on her chin and then several minutes later a slight heaviness in the chest. Patient was transferred to Urgent Care, was treated with Cetirizine Hydrochloride 10 mg at 10:22am, then Epinephrin injection at 10:59 am. Additionally was given 1 Liter of Normal Saline by IV which started at 11:05 am. She was monitored for 1.5 hours and was discharged home with full resolution of symptoms.

Feeling hot Musculoskeletal discomfort Pain in jaw

Symptomtext

PCV 15 given in left arm by this nurse. Patient mmediately stated that she felt a "thunk" while vaccine was injected. Fluzone was then injected to the lateral side of PCV a little lower in the left arm. Again, patient stated that she felt a "thunk' and stated she can feel something "weird " going up her neck, under her left ear and then along her left jaw line to her chin. She states it feels hot, the discomfort is going up in her ear and her ear feels hot. No sob of breath or increased swelling to area noted. NP was notiifed of status immediately and did come to room to examine patient. Patient continued to state that she was ok, it just felt hot. NP advised taking ibuprofen and applying heat to the area. Patient voiced understanding, got up from table. Was able to bear weight without difficulty and denied dizziness or lightheadedness. NP asked her to call back by 5 pm to advise how she was doing and if she should start feeling any different to call immediately call. Patient did voice understanding. at 4:08 PM following injection: Called and spoke with patient. She stated that her left ear no longer felt like it was "burning hot". The area under her left ear under the jaw line to her chin is still sore, but no longer burns or causing her discomfort. She denies any other pain or discomfort

about about 10 min after receiving two vaccines patient felt light-headed, eyes heavy. she returned to IMZ area and like she would faint. we provided aid to help her recover and after about 15-20 min she felt fine.

inadvertently administered ADACEL instead of DAPTACEL to a 16-month-old patient with no reported adverse event; Initial information was received on 13-Dec-2023 regarding an unsolicited valid non-serious case received from a nurse. This case involves a 16 months old female patient to whom inadvertently diphtheria-2/tetanus/5 AC pertussis vaccine [Adacel] was administered instead of diphtheria-15/tetanus/5 AC pertussis vaccine [Daptacel] with no reported adverse event. The patient's past medical history, medical treatment(s), vaccination(s) and family history were not provided. Concomitant medications included hepatitis A vaccine inact (VAQTA) for Immunisation; and HIB vaccine CONJ (TET TOX) (ACTHIB) for Immunisation. On 12-Dec-2023, the patient received a 0.5 ml, once (total) dose of suspect diphtheria-2/tetanus/5 AC pertussis vaccine (Suspension for injection) (lot U7928AA, expiry date: unknown, strength: standard) via intramuscular route in the left deltoid for Immunisation and inadvertently administered adacel instead of daptacel to a 16-month-old patient with no reported adverse event (wrong product administered) (latency: same day). This suspected adverse reaction report is submitted and classified as a medication error solely and exclusively to ensure the marketing authorization holder's compliance with the requirements set out in the Directive and Regulatory Authority Practices. The classification as a medical error is in no way intended, nor should it be interpreted or construed as an allegation or claim made by the marketing authorization holder that any third party has contributed to or is to be held liable for the occurrence of this medication error.

Patient came in for a 5 year well child visit. Patient needed MMRV and DTap for school. Patient was given Tdap (adacel) and Dtap (Quadracel) at 11:15 am 12/4/23. Documentation has been done and trying to notify parents.

Tdap administered in error to a patient with gestational age 12-week 4 day. EDC is 6/10/2024. Would prefer Tdap administered early third trimester. No adverse effect to the med administration. She will not receive the med later in pregnancy.
