Symptomtext

RN identified pt with full name and DOB. Pt was alert and RN administered the flu vaccine. Pt tolerated it well. RN documented the flu vaccine in nurse office. Pt's family was in room and alerted staff to help as pt had become unresponsive. RN went to room and the doctor was there along with other nurses from the unit. Rn was not a part of the code. Witnessed staff attempt to gain vitals, and was unable to get blood pressure. Pt was breathing and had a pulse. A staff member called 911. An ambulance came and transport pt to ED.

Symptomtext

Pt. reports entire arm aching but there has been no soreness, heat or signs of infection. Pt. reports this soreness gets better with Advil or Tylenol. Pt. reports she has been taking the OTC meds every 6-8 hours as needed.

Symptomtext

Pt expressed sweating and dizziness a couple minutes after receiving the flu vaccine. An MA got a BP of 94/56. An NP helped assess the pt and advised it could be a possible vasovagal reaction. Pt reported that she had never had an adverse reaction to the flu and that she hadn't eaten anything this morning. Pt reported that she usually does eat in the morning. Pt reported this reaction happening only once before when she got her blood drawn. RN got water and crackers for pt, took vitals, and took blood glucose. Blood glucose was 71 and BP went back up 110/76 after 15 minutes. Pt still felt dizzy and RN encouraged pt to stay seated and eat more crackers and drink more water. RN retook vitals again after 15 minutes and BP was 109/80. Pt reported feeling better and was able to stand and walk. Pt was given ER precautions that if she began to feel dizzy, faint, or sweating profusely again to call 911 and go to the emergency room.

Symptomtext

Awoken by muscle spasm in both calves. Severe pain and discomfort. Waned yesterday morning to about a 2/10. Last evening severe pain and spasms in both arms and both calves. This morning the same. Had to call out of work (a career firefighter/paramedic) due to severe discomfort. Losing 24 hours of work. Have been taking ibuprofen and using percussion theragun massager with little to no relief.

Symptomtext

Redness, swelling, itching, and warmth to the injection site. Rash has spread over the course of a few days and has become larger each day (3 days). We were asked to monitor the spot and any other symptoms. Treating with cold compresses and Hydrocortisone. If it continues to spread, antibiotics may be used to treat incase of infection.

69 year old patient was supposed to receive fluzone hd qiv but received fluzone qiv instead with no reported adverse event; Initial information received on 26-Sep-2023 regarding an unsolicited valid non-serious case received from a other health professional. This case involves a 69 years old female patient who was supposed to receive Influenza quadrival A-B high dose hv vaccine [fluzone high-dose quadrivalent] but received fluzone qiv (influenza quadrival A-B multidose vaccine preserved) instead with no reported adverse event. The patient's past medical history, medical treatment(s), vaccination(s) and family history were not provided. On 25-Sep-2023, the patient received a 0.5 ml dose of suspect influenza quadrival A-B multidose vaccine preserved Suspension for injection (lot U8049BA, Expiry: 30-Jun-2024) via intramuscular route in the deltoid nos (not otherwise specified) as immunization instead of Influenza quadrival A-B high dose hv vaccine with no reported adverse event (wrong product administered) (Latency: same day). Action taken: not applicable. At time of reporting, the outcome was Unknown for the event. This suspected adverse reaction report is submitted and classified as a medication error solely and exclusively to ensure the marketing authorization holder's compliance with the requirements set out in the Directive 2001/83/EC and Module VI of the Good Pharmacovigilance Practices. The classification as a medical error is in no way intended, nor should it be interpreted or construed as an allegation or claim made by the marketing authorization holder that any third party has contributed to or is to be held liable for the occurrence of this medication error.
